| Backgroud Chronic atrophic gastritis (CAG) is the most common disease managed by GI doctor. It was sure that there is a close relationship between CAG and gastric cancer. WHO named CAG as one type of gastric pre-neoplastic condition in 1978. Suppression or reversion of CAG is thought to be helpful in prevention of gastric cancer, though the results of the taken measures remained confusion. We noted that the researchers in the past had made great efforts on reducing the damage factors for mucosa, while few administrations had been carried out to improve the protective factors. Teprenone is a new type medication, considered to have much better effect on mucosa protection than others. In Japan, it had been reported that teprenone could suppress gastric atrophy induced by short-term administration of N-methyl-N' -nitro-N-nitrosoguanidine in rats. In our study, we prepared CAG model in SD rats induced by ammonia, alcohol and deoxycholic acid, via this animal model we expected to acquire much more information about the suppressive effect of teprenone on CAG.Material and methods 50 SD rats, male, Sexual maturity and healthy, mean weight about 250g, was randomly divided into 5 groups (5 X 10) . 1. Nomal group: fed in regular way; 2. CAG group: 0. l%ammonia, 60%alcohol and 20mmol/Ldeoxycholic acid used for preparing the CAG model. 3. Teprenone group: teprenone 200mg kg-1 d-1; 4.sucralfate group: sucralfate 800 mg kg-1 d-1; 5. NS group (as blank control): natural saline 2ml d-1. Group 3, 4 and 5 were also administrated by 0. l%ammonia, 60%alcohol and 20mmol/Ldeoxycholic acid, which was the same in group 2. At the end of 24 weeks, all the rats were killed. The specimen of the stomach was stained by HE and PCNA, except some part of mucosa was scraped for measurement of hexosamine and PGE2.Results successful CAG model was performed by 0. l%ammonia, 60%alcohol and 20mmol/Ldeoxycholic acid. HE stained pathological outcome: in the antrum of CAG group, the numbers of the gland in mucosa decreased obviously, with disorder of the gland structure and severer infiltration of lymphocyte and plasmocyte; while the muscularis mucosa thickened most and inserted into the substantia propria like braches. The impersonal index showed: the thickness of gland layer and the number of gland in 1 mm of mucosa in CAG group were statistically lower than normal group in gastric antrum; the index of inflammation on mucosa and the wideness of muscularis mucosa were statistically higher than normal both in antrum and body. On the contrary, Group 3 (Teprenone) and Group 4 (sucralfate) had similar manifestation with normal group, even if the both groups were administrated by ammonia, alcohol and deoxycholic acid. Compared with CAG group, Group 3 (teprenone) and Group 4 (sucralfate) had superiority on much wider thichness of gland layer and higher number of gland in antrum. But we determined that there were no obvious deference between teprenone and sucralfate under HE stain. PCNA stained pathological outcome: those groups administrated by stimulative factors all had lowed positive expression of PCNA than nomorl group in antrum . Group 3(teprenone)and Group 4(sucralfate)had statistically higher positive expression of PCNA than CAG group in antrum. Level of hexosamine and PGE2 in mucosa: the level of hexosamine decreased significantly in those group administrated by stimulative factors compared with normal group, while the level of PGE2 had no statistically change. Group 3 (teprenone) and Group 4 (sucralfate) kept their superiority on the level of hexosamine and PGE2. Although there was no statistically deference between those two groups, teprenone had more predominance than sucralfate.Conclusion 1. teprenone had successful suppressive effect on CAG induced by ammonia, alcohol and deoxycholic acid in SD rats. 2.Its correlative mechanisms includes promotion on proliferation of mucosa in protective way and enforced the protective factors of mucosa such as the level of hexosamine and PGE2. |
