The screening methods which is simple, effective, economic, feasible are the blot - spot realm of oncology study, pep-sinogen is the final product which is secreted by matured gastric mucosa cell and it is a label of which digastrics foundations get maturity gradually. According to the characters of immunology and biochemistry, pepsinogen can be devided into two types; PGA and PGC . PG secreted into stomach cavity is activated largely when counter with gastric acid, the smaller go into bloom directly through gastric capillary wall. It is expressed by the study recent years, the concentration of PGA, PGC and the ratio of PGA/PGC was down between the patients of chronic atrophic gastritis and the patients of gastric cancer. But the relation ship between PGC tissue expression and PG serologic concentration should be made a further discuss.The test took advantage of different types of specimen of gastric mucosa biopsy and serum to make histology and serum study, which aim at compare dynamic expressive stale of PGC and the level of serum pepsinogen, in order to estimate their value in screening and diagnosis of gastric cancer.MethodsWe examined 129 cases of gastric mucosa biopsy material and serum specimen of third research department of cancer institute of Chinese medical university, including 30 cases of superficial gastritis,35 cases of gastric ulcer or erosion, 29 cases of atrophy gastritis and 35 cases of gastric cancer. All the speci-mens were identified by pathology diagnosis. There was no significant difference in every of the disease groups in component of sex and age( P >0.05).The two - step SP method of immunohistochemistry was used to examine the expression of pepsinogen C of stomach mucosa; The contain of PGA and PGC in serum were determined by enzyme linked immunosorbent assay (ELISA).Results1. Dynamic expression of various gastric mucosa all the PGC antigens the 30cases superficial gastritis were positive, expressive rate is 100 percent. The positive rate of PGC antigen decrease on sequence superficial gastritis/gastric ulcer or erosion, atrophic gastritis, gastric cancer. Expressive rate of PGC decrease significantly (P <0.01 )from gastric benign disease to gastric precancer-ous lesion; PGC expressive rate decrease gradually (P <0. 05)from gastric pre-cancerous lesion to gastric cancer.2. Comparisons of pepsinogen concentrations in different gastric diseases: Its shown in our test that there was no statistics difference in concentrationof SPGA, SPGC among the groups. The radon of SPGA/SPGC decreased on sequence in superficial gastritis gastric ulcer or erosion, atrophic gastritis, gastric cancer, and there was extreme significant statistics difference (P<0.01) when gastric cancer group compared with superficial gastritis group. There was significant statistics difference between atrophic gastritis group and artificial gastritis.3. Comparisons between different expression of tissue PGC and concentration of serum PG:By the decrease of PGC expression, lesion tissue in superficial gastritis, gastric ulcer or erosion, atrophic gastritis, gastric cancer, their concentration of serum PGC was increased and their SPGA concentration was decreased, but there was no statistic difference. The SPGC/SPGC ratio decreased along with decrease of PGC expression in lesion tissue, to compare with atrophic gastritis and gastric cancer as gastric mucosa benign disease, the superficial gastritis had good relativity and the statistic difference was similar.Conclusions1. Tissue expression of PGC is negative to the malignant degree of gastric mucosa cell and positive with the development of gastric mucosa diseases.2.
