IntroductionGallbladder cancer was one of the most dangerous diseases to mankind and both the morbidity and mortality of which increasingly rose in the world. In China, its mortality was on the fifth top list of digest tumors. Because of the difficulties of the early diagnoses in gallbladder cancer and the occurrence areas of cancer close to the major blood vessel such as portal vein and liver artery, gallbladder cancer is easy to invade liver parenchyma , pancreas and major blood vessel of liver gateway and has high levels of malignant tissues. Although interventions such as operation were used, there was no obvious improvement in prognosis, for the development of Gallbladder cancer was a complicated course with multi - gene change. So, the further researches of molecular biological feature were important to the diagnosis, therapy and prognosis of gallbladder cancer.Tumor was a kind of diseases mainly resulted from cells mutation. Further researches showed that there was a close relationship between tumor agenesis and the control of abnormal cell death. So, apoptosis and tumor became a new focus in molecular biological researches. Survivin found recently was a novel inhibitor of the inhibitor of apoptosis protein (1AP) , the distribution of which had a high selectivity, and there was no survivin expression in normal tissues ( except pectoral gland and genital gland) but in many tumors, which was different from another antiapoptosis gene bcl -2. In spite of the difference of expression levels, this study of survivin may take us a pathway to diagnose and treat tumor. At present, the reports about survivin expression in Gallbladder cancer were ab-sence and the correlation between its expression and clinical pathology wasnt clear. This study emphasizes to investigate the expression of antiapoptosis gene survivin in gallbladder cancer and the relationship with the expression of P53 and bcl - 2.Materials and methodsMaterials54 samples during November 2001 to April 2002 were collected in the department of general surgery of the First and Second Affiliated Hospital China Medical University. There were 16 male, 38 female patients with an age from41 to 78 years old and 34 cases of gallbladder cancer, 10 cases of benign gallbladder polypi. According to Nevin stages, 34 cases of gallbladder cancer were classified as 1 case stage I, 14 cases II , 11 cases III , 3 cases IV and 5 cases V. According to pathological subtype, there were 33 cases of gland tumor and 1 case of gland - squamas carcinoma. According to the differentiating degree, the cases were classified as highly differentiated (23cases) and lowly differentiated (11 cases) . There were 12 cases with lymph metastasis, and 13 cases with gallbladder calculi. No cases were received rediotheropy or chemiotheropy before operation.Methods1. Immunohistochemisty for survivin, P53 and bcl -2 protein Immunohistochemical tests were performed by SP method. The SP Kit wasproduced by FuZhou Maixin Biological Corp. China. The primary antibodies were monoclonal mouse antibody against survivin ( 1;30) , mouse monoclonal antibody against P53 and mouse anti - human bcl - 2, and all the procedures were followed by the instruction. The negative control was prepared in the PBS, the positive control for staining was obtained from the sections of liver cancer. The positive were defined as brown to yellow staining in cytoplasm and nucleus.2. Statistical analysisChi - squared test was used in data of quantity. The data were analyzed by the SPSS software.Results1. The expression of survivin mRNA: 76% in gallbladder cancer (21/ 34). There was no expression in benign gallbladder polypi and chronic cholecystitis.2. There was no significant correlation between positive survivin expression and age, sex, calculus, histological subtype, clinical staging, lymph node metastasis.3.
