ObjectiveOvarian cancer is the common malignant tumor which seriously imperils health of women, the morbidity is on the rise and the death rate hold primacy in gynecologic tumor. The 5 - year survival is only 30%. It is difficult to be found and diagnosed early because ovarian is hidden in the pelvic cavity and isnt of specific symptom. As result, many patients can be diagnosed till they have asci-tes on the late stage. Up to date, patients with advanced - stage disease are still difficult to be completely cured by systematic and normal measures because we cant remove all focus of infection and lack ideal chemotherapy. Recurrence is one of its factors. It is very important for us to screen after treatment and find early any signs of epithelial ovarian cancer recurrence. At present, measurement of serum CA - 125 levels, physical examination, ultrasound, CT and MRI are used to follow - up methods for ovarian cancer. Measurement of serum CA - 125 levels is the most commonly used because of its higher specifity. CA - 125, which is defined by the mince monoclonal antibody OC -125, is associated with high - molecular - weight glycoproteins. It is expressed by most epithelial ovarian cancers. Measurement CA - 125 plays an important role in the course of o-varian tumor diagnosis and following - up since 1981. Many authors between nation and international are paying attention to its clinical value in diagnosis and monitoring of disease.The aim of the present study is to evaluate the relationship between serumCA -125 and the recurrent factors of epithelial ovarian cancer by analysis.Materials1. In the period from January 2000 to May 2003, a total 75 patients after treatment and monitoring for invasive epithelial ovarian cancer have been regist-ed. According to FIGO criteria, Stage I + II 22 cases, ffi + IV 53 cases. Se-rous(n=41), Endometrioid(n = 13) , Mucinous(n = 19). Clear cell (n=2). Residual disease >2cm (n =23), ^2cm (n =52). Including 15 patients in uncontrolled, 30 patients in recurrent, 30 patients in remission. Rate of recurrence is 50% (30/60). Mean age of recurrent patients is 52year (range.- 32 -75year) and mean recurrent time is 12.5 month (range; 2.5 - 33month). On the primary operative report, standard surgical procedures were total abdominal hysterectomy, bilateral' adnexectomy, omentectomy, appendectomy and lymph nodes which were palpated. After operation, patients are accepted CBP or DDP chemotherapy and followed -up. Of 30 recurrent patients, there were Serous (n = 17) , Endometrioid(n = 5) , Mucinous( n = 8). According to FIGO criteria, Stage was I + II in 3, I in 24, IV in 3.Methods1. Because criteria of recurrent epithelial ovarian cancer isn' t explicit, we were according to the criteria made by Lian Li Guan. ( 1) Remission: serum CA- 125 level reduces to normal and residual disease isnt observed by clinic and physical examination after primary treatment. (2) uncontrolled : serum CA -125 level doesnt reduce to normal, but rises again after primary treatment. (3) Recurrence: residual disease is observed by clinic and physical examination or serum CA - 125 level rises again after remission. (4) The half - life of serum CA125 (t1/2) =t/21og(CA,/CA2).2. Clinic materials were collected. Serum CA -125 was tested by the IM-MULITE OM - MA assay. The results ranged from 1.9 to 16. 3 U/ml. 2.3 Statistical analyses; Statistical analysis was performed using SPSS10. 0. Dates werecompared by X , Nonparametric Test and Correlation analyses are performed by the Spearman rank test, Logistic. P <0. 05 was required for statistical significance.Results1. Influent factors of epithelial ovarian cancer: 1. 75 patients were divided into three groups which were uncontrolled, recurrence and remission. Patients in stage I, IV were 93. 3% ,90% ,40% , and serous was 86. 7% ,56. 7% ,36. 7% , respectively.
