Immunological And Electrophysiological Study Of Anti-Hu-Associated Paraneoplastic Syndromes Of Nervous System

The role of specific neuronal anti-Hu antibody in paraneoplastic syndrome of nervous system (PNSNS) and primary tumor, especially small cell lung cancer (SCLC) was investigated by immunohistochemistry ABC methods and western blot; The character of Sub-clinical neuropathy with patients with only SCLC was studied by electrophysiological techniques such as EMG, NCV and SEP. The main results as follows:1. Sera immunological features of anti-Hu associated PNSNS:1.1 The sera of 99%( 19/20) patients with PNSNS reacted with the frozen sections of human cerebra and stained with indirect immunoperoxisase methods at the final dilutions of 1:1 000 to 1:64 000; The sera of 33(33/120) patients with only SCLC reacted with the frozen sections of human cerebra and stained with indirect immunoperoxisase methods at the final dilutions of 1:1 000 to 1:8 000 except a serum sample with the final dilutions of 1:1 000 to 1:32 000 ;1.2 With western blot ananlysis the specific neuronal anti-Hu antibody sera identified several bands of approximately 35 to 40 KD of nuclei exact and 40 KD of HuD clone protein ,the anti-Hu antibody was detected in 19 patients sera of 20 patients with PNSNS ;The anti-Hu antibody was detected in 2 patients sera of 120 patients with NPSNS.1.3 The anti-Hu antibody was not detected in the 32 normal human sera.2. The early diagnosis value of specific neuronal antibody in small cell lung cancer: 2.1 The sera of 34 patients reacted with the frozen sections of human cerebra andstained with indirect immunoperoxisase methods ;the sera of 56 patients no reacted ;2.2 72 patients were diagnosed as SCLC ,18 were as NSCLC by the results of celluar and pathological study; The ABC positive rate of SCLC patients is 43% or so;The positive rate of NSCLC patients is 16% or so;There is different postive rates between SCLC group and NSCLC group;2.3 The ABC positive rate of normal control group is zero.3. Specific neuronal anti-Hu antibody for use in the early diagnosis of SCLC:3.1 The sera of 40%(32/80) patients of only SCLC without PNSNS reacted with the frozen sections of human cerebra and stained with indirect immunoperoxisase methods at the final dilutions of 1:1 000 to 1:8 000;the sera of 89% (16/18)patients of SCLC with PNSNS reacted at the final didutions of 1:1 000 to 1:64 000 ;3.2 With western blot ananlysis the specific neuronal anti-Hu antibody sera identified several bands of approximately 35 to 40 KD of nuclei exact and 40 KD of HuD clone protein ,the anti-Hu antibody was detected in only two patients sera of only SCLC without PNSNS, and 16 of SCLC with PNSNS group ;4. The character of Sub-clinical neuropathy with patients with only SCLC:4.1 The most common findings in electrophysiological studies from 4 patients of 10 with only SCLC is reduced or abolished sensory potentials associated normal of motor nerve conduction, abnormal changes of SEP;4.2 No abnormal electrophysiological changes was appeared on 6 patients of 10 with only SCLC .The results revealed that:1. Specific neuronal anti-Hu anti-body is a specific and sensitive marker in detecting paraneoplastic syndrome of nervous system(PNSNS) and play a important role in immune disorders;2.There are probably no necessary relations between PNSNS and anti-Hu at high liters, cellular immune response may have a effect on PNSNS;3.Specific neuronal anti-body, especially specific neuronal anti-Hu antibody may be a new factor for use in the early diagnosis of SCLC ;4.Subclinical neuropathy was discovered from patients with only SCLC , it may be a tranditional conditions between normal conditions and PNSNS.
