Objective: To constitute an exact and impersonal method to detect the cytokines in T lymphocytes and discuss the changes and meanings of T-lymphocyte subsets and expression of Th1,Th2 correlative cytokines in children with idiopathic thrombocytopenic purpura(ITP ) . Method: We examine 18 adults, 10 healthy children and 19 patients with ITP using flow cytometry and monoclonal antibody . The lymphocytes in whole blood are stimulated with PMA and lonomycin, Which can stimulate the cytokine synthetic potential of each cell. Then Monensin can retain cytokines within the lymphocytes. Anti-CD4 antibodys and anti-cytokine antibodys are used to label antigens respectively. And flow cytometry is employed to determine and analyze. At same time, we detect their T-lymphocyte subsets. Result: The expression of cytokines IFN-r, IL-4 and T-lymphocyte subsets in healthy children is consistent to those of adult. But the expression of IFN-r in patients with ITP is higher than those in healthy children, and the expression of IL-4 is greatly lower than those of healthy children. Otherwise the children with ITP have lower CD4+-cells, higher CD8+-cells and lower CD4+ ,CD8+ cells. Conclusion: This technique allows a more accurate and detailed comparison of cytokine profile at single-cell level than in the past.The patients with ITP have disorders of T-lympocyte subsets and abnormal expression of cytokines . There is an abnormal profile of Th1,Th2 from children with ITP. And these disorders may have significant meanings in mechanism of ITP.The research for abnormal expression of cytokines may have some importance.
