A Clinical Study Of Zenapax In The Orthotopic Liver Transplantation

Objective:To observe the efficacy, safety and influence of function of allograft that 3 Doses of Zenapax combine with Tacrolimus, Mycophenolate Mofetil, and Steroid in the early stage of liver transplantation. To study the concentration of tacrolimus in blood when combined triple immunosuppressive agents.Method:13 cases of liver transplantation recipients received the same immunosuppressive drug regimen which 3 Doses of Zenapax Combined with Tacrolimus, Mycophenolate Mofetil, and Steroid. The happening of acute allograft rejections, the concentration of tacrolimus in blood, the dosages of Tacrolimus and Steroid, the counts of CD3+, CD4+, CD8+ and CD25+ , and the transplanted liver function were observed in the 30days after liver transplantation.Results:1. One AR occurred in 13 cases of liver transplantation recipients.2. No anaphylaxis, PTLD, and dysoccurrences were observed.3. 10 recipients suffered from various kinds of infection. For them, 9 cases with pulmonary infection, 6 with abdominal cavity infection, 4 with infection of biliary tract. 7 recipients with the infection above two systems. No infection of CMV, EBV, andtuberculosis weren' t found.4. The dates that the function of transplanted liver became normal were 16.56 + 10.40 days.5. 3 cases died in 13cases of recipients. The causes of death were pulmonary fungal infection, acute hepatic failure from multiple organ failure as stenosis and thrombus of hepatic artery respectively. The times of death were 14, 19, and 23 days posttransplantation.6. The counts of CD25+ could be controlled for 30days after transplantation of live by 3 doses of Zenapax.7. The concentration of Tacrolimus is 10.25 + 1.99ng/ml, the dosages of Tacrolimus and Steroid respective are 5 + 2mg/d and 2. 65 + 0. 47gConclusion:1. 3 Doses of Zenapax combined with Tacrolimus, mycophenolate mofetil, and steroid after orthotopic liver transplantation decrease and postpone the occurrences of AR, and relieve the degree of it.2. The graft function weren' t damaged by 3 Doses of Zenapax combined with tacrolimus, mycophenolate mofetil, and steroid. The recovery times of the transplanted liver function were 16. 56 +10. 40 day.3. 3 Doses of Zenapax combined with Tacrolimus, mycophenolate mofetil, and steroid after orthotopic liver transplantation were efficacy, safe, reasonable and suitable in the 13 recipients.4. In the therapy method, the concentration of tacrolimus in blood is 10.25 + 1.99ng/ml, the dosages of tacrolimus and Steroid respective are 5 + 2mg/d and 2. 65+0. 47g.5. The use of 3 doses of Zenapax is simple and convenient.
