BACKGROUND: Intestinal Obstruction can be brought by many causes and it is one of common diseases in surgery practices. It not only can elicit topical lesion in alimentary tract, but also can lead to the systemic physiological disorder, translocation of bacteria and endoxin from intestine into systemic circulation, which can elicit series of reactions in mononuclear phagocyte system, then a great amount of associated intermediate and cytokine will be released. Sometimes multiple organ dysfunction syndromes (MODS) may happen, and induce the patients' death ultimately. Dachengqi mixture is the Chinese traditional Tongligongxia herbs, the practices prove that Dachengqi mixture have a good effect in treating incomplete intestinal obstruction. Many acute abdominal pains often company with the blood stasis. Huoxuehuayu herbs can activate blood circulation, disperse stasis and strengthen the body's immune function. But it needs to be proved that the huoxuehuayu herbs have the other intensive effects. OBJECTIVE: To study the huoxuehuayu herbs can enhance the effect of tongligongxia herbs in treating incomplete intestinal obstruction. METHODS: (1) Model Establishment: The 1/2 terminal ileum of rat was ligated as incomplete intestinal obstruction model. (2) Groups: Rat was randomly divided into 6 groups: normal group, sham operated group, model group, dachengqi mixture group, fufangdanshen mixture group and huoxuechengqi mixture (combine dachengqi mixture and fufangdanshen mixture) group. Half-hour before the operation, the herbs (1.06g/100g) was given to the corresponding therapeutic groups, and then was continued at the same time in the morning for three days. Saline was given to the rats of normal, sham operated and model groups according to the same method. For the model and therapeutic groups, the intestinals were made pinprick and clamped. For the sham operated group, the intestinals were only made pinprick. The normal groupdidn't do operation. (3) Items of Observation: plasma NO, MDA, SOD, plasma and tissue histaminase, [Ca2+] of the Intestinal epithelial cells (IEC) and the quantities of fluorescence labeling E.coil in liver, spleen and lymphoid. RESULTS: (1) After given dachengqi mixture or huoxuechengqi mixture, plasma histaminase decreased (P<0.05 or P<0.01). Given dachengqi mixture, fufangdanshen mixture or huoxuechengqi mixture, plasma NO (P<0.01), [Ca2+] of the Intestinal epithelial cells (P<0.05 or P<0.01) all decreased significantly, but histaminase of tissue increased remarkably (P<0.05 or P<0.01). Compared with the dachengqi mixture group, huoxuechengqi mixture group had significant difference (P<0.05 or P<0.01). (2) Given dachengqi mixture, fufangdanshen mixture or huoxuechengqi mixture, plasma MDA (P<0.01), the translocation rate and quantities of E.coil (P<0.05) all decreased, but SOD of plasma (PO.01) and tissue (P<0.05) increased. Compared with the dachengqi mixture group, huoxuechengqi mixture group hadn't significant difference. (3) Plasma NO, MDA and histaminase were positive correlated with [Ca2+] of the Intestinal epithelial cells, but plasma SOD was negative with it (P<0.01). CONCLUSION: (1) The model was successful. After intestinal obstruction 72 hours, the NO of plasma, MDA, histaminase of the plasmal all decreased, but SOD of the plasma and tissues were increased. (2) Dachengqi mixture, Fufangdanshen mixture and Huoxuechengqi mixture can all clear liqid peroxide free radical, reduce translocation of E.coil and protect the intestinal barrier at different degrees. (3) Probably the intestinal barrier function correlates with decreasing [Ca2+] of the Intestinal epithelial cells to lessen the intestinal epithelial cells injury and decreasing the release of histaminase in the Intestinal epithelial cells. (4) Fufangdanshen mixture can enhance effects of Dachengqi herbs in treating incomplete intestinal obstruction. |
