Objective:To learn the etiology of hydrocephalus,explore the indication of endoscopic third ventriculostomy and compare the postoperative effect and complications of two methods for obstructive hydrocephalus via the clinical analysis. Methods:32 patients who underwent endoscopic third ventriculostomy and 41 patients who underwent ventriculo-peritoneal shunt operation for occlusive hydrocephalus were followed prospectively from January1998 to December2003. Results: In ventriculostomy group,the medial operating time was 40 minutes,30(93.7%)improved and 2(6.2%)recurred,3(9.4%)had complications as fever,diabetes insipidus.In shunt group,the medial operating time was 76 minutes,41(100%)improvedand10(24.5%)recurred,11(26.8%)hadcomplications included tube blocked, infection,intracranial hematoma or trapped fourth ventricle TFV. Conclusions:Endoscopic third ventriculostomy has equal effect compared with ventriculo-peritoneal shunt operation for obstructive hydrocephalus, and it can decrease the operating time, complications. So ventriculostomy should be the first selection of occlusive hydrocephalus as shunt alternatives. |
