Tissue doppler imaging ( TDI ) which includes velocity imaging, strain rate imaging, strain imaging and so on can quantitatively analyse the regional myocardial function precisely. It is supposed to be used to appraise the serious level of coronary artery disease ( CAD ) and to observe the therapy result. But the discipline of how the TDI indices change in the different cardiac segments in healthy people or CAD patients has not been disclosed clearly until now.This research will reveal how the TDI indices change in the different segments of the left ventricle in healthy human being. If or not there is significant difference between the ischemic group and the control group also will be analysed, and the most sensitive ischemic index will be identified. The change of the TDI measurements in the infarction regions, in the neighboring regions and in the remote regions will be compared. The regional myocardial function after PTCA will be observed and the best time to value the final curative effect of PTCA will be ascertained. This research includes four parts:l.The discipline of how tissue velocity, strain rate and strain change between the different segments of the left ventricle in healthy human beingObjectives: To disclose how the TDI indices changes in the healthy people's left ventricle and if the velocity, strain rate and strain have the similar discipline. To find out the relativity between the TDI indices and the age.Methods: Thirty-eight healthy people were examined by echo-cardiography. The apical four chamber view and apical two chamber view were stored in the form of TDI two dimensional color images. The left ventricle septum, anterior wall, lateral wall, posterior wall were analysed, which were divided to basal, middle and apex segments respectively.Every segment's velocity, strain rate in the systolic, diastolic and atrial systolic periods were measured. The maximal strain was determined too. Then the difference between the different segments of the same wall and between the different walls of the homonymous segment were detected. Besides, the relativity of the TDI indices and the age were studied.Results: In every wall of healthy left ventricle, from basal segment to apex, the TDI indices had the same decline trends. The difference was significant between the different segments' velocity, but it was not significant between the strain rate and strain. There was no difference of velocity, strain rate and strain between the four walls. The age is elder, the absolute velocity of systolic and diastolic periods is lower. No linear relativity was found between the age and the velocity of atrial systolic period, the strain rate or strain.Conclusions: From basal segment to apex, the velocity declines, but the strain rate and strain are same. The velocity, strain rate and strain are same among the different walls of the left ventricle. There is negative relativity between age and velocity, but there isn't linear relativity between age and strain rate or strain. The train rate and strain are more believable than velocity when evaluate the regional myocardial function between the people whose age are not same.2.The change of the tissue velocity, strain rate and strain in angina pectoris patientsObjectives: To observe whether the regional myocardial velocity, strain rate and strain have significant difference between the angina group and the control group.Methods: Fourty-one angina pectoris patients were compared with 31 healthy people to find out if there was significant decrease of the TDI indices, and which one can suggest ischemia earlier and more sensitively.Results: The angina pectoris patients' ischemic segments had the decreased TDI measurements although their constriction was normal observed by the naked eyes. The segments whose strain rate decreased were less than that ones whose velocity or strain declined.
