In this project, an experiment was conducted to study the effect of Yishou Tiaozhi Tablet (YSTZT) on glucose metablism in diabetic animals, and a clinical survey of its efficacy on lipid metabolism in hyperlipidaemia was also investigated.Experimental StudyMethods: 70 SD rats were randomly assigned to controls, diabetes, metformin, low dose YSTZT and high dose YSTZT five groups. 60 were under abdominal cavity injection with 5% tetraoxypyrimidine for diabetic modelling and the controlled group was injected with an identical amount of normal saline. Diabetic rats were defined by the fasting blood glucose level of more than 16.0mmol/L and the specimen was collected from the tail vein one week after the injection. Three weeks after drug feeding, a number of experimental outcomes were measured which includes the rat body weight, fasting blood glucose, whole blood viscosity, plasma viscosity, serum NO and plasma ET.Results: Comparing with the diabetic group, YSTZT significantly increased the rat body weight (p<0.01) and reduced the blood glucose significantly (p<0.01). High dose YSTZT group had similar blood glucose lowering effect as metformin group. Although both dose YSTZT improved whole blood viscosity and plasma viscosity, high dose YSTZT showed a much better outcome (p<0.01). Both dose YSTZT groups significantly increased serum NO level and reduced plasma ET level significantly among diabetic rats (p<0.01).Conclusion: YSTZT is effective in lowering blood glucose. Its mechanism of lowering blood suger may be explained by improving blood viscosity, intervening serum NO and plasma ET.The actual mechanism of its increasing NO and reducing ET in diabetic rats needs to be further investigated. YSTZT may be viewed as a proper Chinese herbal medical formula for Diabetic Mellitus.Clinical StudyMethods: 118 patients with primary hyperlipidaemia were randomly assigned to YSTZT treatment group, and one of the two simvastatin controlled groups (simvastatin 5mg and 20mg). The levels of a number of serum lipids were measured before and after 60 days of drug treatment.Results: (1) YSTZT group showed a total effective treatment rate of 97.1% while it was 86.8% in simvastatin 20mg group and 86.7% in simvastatin 5mg group. The difference was not statistically significant (p=0.059). (2) The comparison of the effective rate on TC, TG, HDL-c and AI between YSTZT group and the two simvastatin controlled groups did not show any statistical difference (p>0.01). The rate was 83.9%, 77.8%, 33.3% and 68.6% respectively in YSTZT group. (3) A similar treatment effective rate was shown (p>0.4) when comparing between YSTZT group and the two simvastatin controlled groups in combined hyperlipidaemia, hypercholesterolaemia alone and hypertriglyceridaemia alone. (4) Comparing with the baseline data, YSTZT significantly decreased serum TC, LDL-c and ApoBlOO levels by 21.9%, 24.4% and 11.4% (p<0.05) and significantly increased ApoAI/ApoB value by 14.3% (p<0.05). It also showed statistical difference on reducing serum TC, LDL-c and ApoBlOO levels in simvastatin 20mg group (19.7%, 25.4% and 5.3% respectively) while only the serum TC and LDL-c levels showed the difference in simvastatin 5mg group (15.9% and 21.3% respectively). YSTZT produced a better outcome than simvastatin 20mg on decreasing serum TC and TG It had a similar effect on lowering serum LDL-c when comparing with the two controlled groups.YSTZT was not better than simvastatin 5mg and 20mg on increasing HDL-c level but it generated a better result than the two simvastatin groups on decreasing ApoB100, and its effect on ApoAI/ApoB value was superior to simvastatin 5mg. For the effect on lowering TC/HDL-c and AI, simvastatin 5mg produced a better outcome. The YSTZT group showed a similar outcome on lowering serum TC, TG, TC/HDL-c, LDL-c, AI and ApoB100 in combined hyperlipidaemia, hypercholesterolaemia alone and hypertriglyceridaemia alone to the two simvastatin groups.
