Objective: To study the blood supply of rectal cancerswith transrectal color Doppler flow image (TR-CDFI),transrectal pulse wave Doppler (TR-PW), transrectal colorpower angiography (TR-CPA), hemodynamic parameters wererecorded, tumor vessels quantified with TR-CPA were comparedrespectively to their pathology and microvessels density (MVD)derived from immunohistochemistry. The aim was to analyzethe characteristics of blood supply and hemodynamics in rectalcancer, explore the vessels pathologic basis of transrectalDoppler ultrasonography (TRDUS) in detecting and quantifyingtumor vessels, evaluate their clinical value in choosing theoperation methods and assessing prognosis of rectal cancer.Methods: In this study, 63 cases of rectal cancers wereexamined with TR-CDFI, TR-PW and TR-CPA before operation.All of the patients defecated one hour before examinations.They were examined in left lateral keen-breast position. At first ,2D grey-scale sonography was used to depict the position, size,outline, inner echo, invade deep, the adjacencies rounding ofrectal cancer and the number of lymph node around the rectum.The blood supply was showed with TR-CDFI, and the bloodflow distribution was observed. The optional PW spectra wereacquired with sample volume 2mm, angle between the sampleline and blood flow less than 30°. The peak systolic velocity(PSV) , end diastolic velocity (EDV) and resistant index (RI) ofintratumor vessels were measured and their averages wererecorded. The blood flows in the tumors were displayed withTR-CPA and graded into I ~ III according to the blood flowdistribution and degree of blood supply richness. The meancolor vessels density (MCVD) was calculated using AutoCAD2004. It was a quantitative index. All the slices of tumortissue specimens excised operatively were performed HE statingpathologic examine and immumohistochemistry usinganti-CD105 monoclonal antibody and anti-factor-VIII relatedantigen (F8RA) polyclonal antibody. The MVDs werecalculated separately. According to the TNM standardestablished on UICC in 1997, the 63 cases of rectal cancer werestaged I~IV. The results above were analyzed statistically.Results:1 PSV of 63 cases of rectal cancer with TRDUSpreoperational were ( 21.04 ±11.53 ) cm/s, EDV were ( 6.09±2.74 ) cm/s, RI were ( 0.64 ±0.01 ), MCVD were ( 0.22±0.09 ). In 63 cases of rectal cancer, 9 cases (14.29%) of 63were TR-CPA I, 23 cases (36.51%) were II, 31 cases (49.20%)were III.2 The pathological results of the 63 cases of rectal cancershowed that 43 were ductal adenocarcinomas, 12 weremucinous adenocarcinomas , 4 were papillary adenocarcinomas ,2 were signet-ring cell carcinomas , and 2 were squamousadenocarcinomas.3 Immunohistochemically, the membrane of the vesselendothelial cell (VEC) in rectal cancer tissue were stained withanti-CD105 monoclonal antibody and anti-factor-VIII relatedantigen polyclonal antibody, assessing the MVD were(35.81±10.54)/400×and (22.65±7.90)/400×respectively. TheMVDs of rectal cancer estimated with the two markers showedpositive rank correlations (rs=0.6689, P<0.001). The MDVCD105was significantly higher than the MVDF8RA(P<0.05).4 There was negative rank correlation between RI andMVDCD105 (rs=-0.4225, P<0.01). There was positive rankcorrelation between MCVD and MVDCD105 (rs=0.8529,P<0.01).5 Analyzed the relation of RI, MCVD, MVDCD105 andTR-CPA stage, TNM stage: There were statistical diverse of RIin different groups (TNM stage I and III, I and IV, II and IV)(P<0.05). There were statistical difference of MCVD andMVDCD105 in all TNM stages (P<0.05). There were significantdifference of MCVD and MVDCD105 in all TR-CPA grades(P<0.05).Conclusions:1 CD34 and F8RA were expressed in the rectal cancertissues, while CD105 was superior to F8RA.
