Acute coronary syndrome (ACS) is the most common coronary heart disease and has close relationship with the unstable state of artery atherosclerosis (vulnerable plaque). Vulnerable plaque is a atherosclerosis plaque that is easy to thrombosis or may progress rapidly to culprit plaque Irreversible myocardial injury or infarction may happen once the plaques break and thrombosis ,coronary angioplasty already is an effective way to treat ACS, but with the reperfusions ,the ischemia/ reperfusion injury (IRI)decrease the benefits. Up to date, it is believed that inflammation and apoptosis play important parts in vulnerable plaque and ischemia /reperfusion injury, which is one of hot points in cardiovascular diseases recently.IL-18 is an important proinflammatory cytokine found in recent years and has been reported that it may be involved in and play important roles in immune and infectious diseases. IL-18 exists as an inactive precursor and requires cleavage of its precursor by caspase-1. Caspase is the crucial part in cell apoptosis by starting and participating in the apoptosis cascade. Socaspase and IL-18 may be important in vulnerable plaque and IRI.This trial is to investigate the IL-18 changes in coronary heart disease and study its significance, and then the rats of myocardial IRI were used as models to study changes of the serum concentration of interleukin-18 in ischemia/reperfusion - the effects of the interventions of caspase-1 inhibitor on interleukin-18 and apoptosis.Part I: patients with coronary heart diseases(105 cases) and controls with normal coronary angiography (CAG) (22 cases), IL-18 was measured with enzyme-linked immunosorbent assay(ELISA)to investigate the changes with different kinds of coronary heart diseases; high sensitive C-reactive protein (hs-CRP) was measured with immunonephelometric assay to see its changes and the relation between hs-CRP and IL-18.The results show: d)The serum levels of IL-18 and hs-CRP were significantly increased in AMI and UAP in comparison with SAP and control (.PO.01); ?unlike IL-18, the serum concentration of hs-CRP was higher in patients with AMI than those with UAP (P< 0.05). ?The serum concentration of IL-18 was correlated with hs-CRP in patients with AMI.Part II: 60 healthy SD rats were divided into six groups randomly, The rats of myocardial ischemia/reperfusion were used as models to study :(l)changes of the serum concentration of interleukin-18 in ischemia/reperfusion; (2) the effects of the interventions of Z-WEHD-fmk on interleukin-18 ;(3) we also observed the change of structure and shape in the myocardial cells with HE, observed the change of ultrastructure in the myocardial cells with transmission electron microscope;(4) deciding the ischemia area at risk and infarct size by Evans blue solution and TTC dyeing and studying the effects of the interventions of Z-WEHD-fmk; (5)detecting...
