| ObjectiveDiabetic retinopathy(DR) is one of the serious complication of microvascu-lar changes in diabetes mellitus ( DM ). So the research about DR is an important issue to prevent blindness.Up to now, the pachogenesis of DR is till unclear. On the basis of study, it is shown that the development and progress of DR is the result decided by multi-factors and multiphase acting cooperatingly. Recently the effect of leptin is more and more highlighted. Leptin is produced by adipose tissue and deleted by kidney. The biological effect of leptin is developed through its receptor - leptin receptor (Lep — R). According to the clinical discovery, leptin is associated with a series of risk factors to the development and progress of microvasculor complication of DM. Researchers show that serum leptin level increase gradually with the progress of DR. These indicate that leptin has close association with DR, and it plays an important role in the development of DR.In this study, we use immunohistological technique to detect the expression of long receptor - leptin effecting receptor in epiretinal membranes ( ERM ) of patients with proliferative diabetic retinopathy (PDR). And we use ELISA technique to test the concentration of leptin in vitreous both of patients with PDR and of normal people. In this way , we may furthur discuss and demonstrate the action of leptin on the development and progress of DR.MarterialERM of PDR are 31 cases. Among them, 11 cases are in the fifth phase,20 cases are in the sixth phase. Normal retina are 6 cases. Vitreous of PDR are 15 cases. 7 cases are in the fifth phase, 8 cases are in the sixth phase. Normal vitreous are 6 cases.MethodWe use immunohistological technique to test the expression of long leptin receptor in ERM of patients with PDR and normal retina. And use ELISA technique to measure the concentration of leptin in vitreous of patients with PDR and normal people.Statistic management: The comparision of positive rate of leptin receptor expression in the fifth phase and sixth phase is by X testing. The comparision of the concentration of leptin in vitreous of patients with PDR and normal people is by t testing. The comparision of the concentration of leptin in vitreous of patients with PDR in the fifth and sixth phase is by t testing.ResultLeptin receptor is expressed in ERM of patients with PDR, no statistic meaning on positive rate between the fifth and sixth phase. The concentration of leptin is significantly increased in vitreous of patients with PDR compared with normal people. The difference has statistic meaning. The difference of the concentration of leptin between the fifth and sixth phase of patients with PDR has no statistic meaning.DiscussionThe dysfunction, sclerosis and block of the capillary on the retina of DR are the fundemental reasons which lead to blood circulatory disorder. The appearance of new blood vessel is the characteristic pathological change of PDR.
