Objective: To analysize the treatment effects of rotating Gamma knife radiosurgery on primary trigeminal neuralgia. Investigate the relations among factors relating to the efficacy .Methods: From September 1997 to March 2004, primary trigeminal neuralgia in 54 patients with pain refactory to medical management, and other surgical procedures had been treated with Gamma knife radiosurgery in our unit.There were 28 males and 26 females , and their ages ranged from 27 to 80 years, with an average of 62.5 years. The pain appeared on the left for 34 patients , 20 on the right. There are 6 patients' pain from branches 11,2 in branches III, 40 in branches II + III, 6 in all the branches . The target was the CN V entry root zone into the brainstem by use of OUR-RGS/Aγknife. The target was defined by MRI. The dose was delivered to one isocenter with the 4-8 mm collimator.. Radiation doses ranged from 70 to 90 Gy at the maximum dose point.Results: all patients had been followed up from 2 to 72 months.Median follow-up was 37.5 months. Results wereclassified as excellent (no pain, no medications), good (no pain, minimal medication without side effects), or failure(no relieved pain, or less). The relevant numbers are 36, 10, and 8 respectively. Pain recurrence occurred in 4 patients 2-3 years later. The whole efficiency rate is about 85.2%. no any radioactive edema or side-effects are founded.Conclusions: the treatment of rotating Gamma knife radiosurgery on primary trigeminal neuralgia , especially which is refactory to medical treatment, is an effective and safe style.
