Objective: To investigate the expression of MDR1 gene in peripheral blood of patients with intractable epilepsy, the role of antiepileptic drugs (AEDs) and seizures in multidrug resistance of intractable epilepsy, and the genetic polymorphism of C3435T in MDR1 gene.Methods: The one-step semi-quantitative RT-PCR technology was used to test the expression of MDR1 gene mRNA in peripheral blood of 120 subjects, which was classified (Two factors—seizure and AEDs were included according to the statistical method of univariate analysis of variance) as intractable epilepsy in 30 ( group A), drug responsive in 30( group B), seizures without drugs in 30( group C), and 30 control subjects ( group D). At the same time, C3435T polymorphism in group A and D was analyzed respectively by PCR-RFLP.Results: The marked difference of MDR1 gene expression level was seen between the four groups according to the statistical method of univariate analysis of variance (F=4.456, P=0.005). The expression level of MDR1 gene was elevated because of the factor seizure(F=10.005, P=0.002), not AEDs (F=0.919, P=0.340), the interaction of the two factors had no statistical significance (F=2.445, P=0.121) .Of the 30 subjects, 3 (10%) were C/C genotype, 9 subjects (30%) were C/T genotype, and 18 subjects (60%) were T/T genotype in C3435T of group A.
