| Endometrioid Adenocarcinoma is one of the most commonly seengynecology malignant tumor, epithdal-cadherin (E-cad) has theeffect of promoting the coherence of cell-cell, maintaining thecompleteness and polarity of constitution. E-cad is closely relatedto bata-catenin (β-cat), β-cat has double function: it is thedownstream component of thoroughfare for signalling pathways: thesignalling pathways extraordinary activation of wnt interposed byβ-cat is closely related to the genesis and development of tumor.At present, there is no report on the study of united detect inEndometrioid Adenocarcinoma for E-cad and β-cat. This studyadopts the technology of immunohistochemistry to examine theexpression of E-cad, β-cat, in different types of endometrium andcarcinoma of endometrium in order to clarify the carcinomatouschange mechanism of Endometrioid Adenocarcinoma.Take 37 paraffin cases of Endometrioid Adenocarcinoma, which havebeen diagnosed after operation from First Affiliated Hospital JiLinUniversity during 1999 and 2003, all of the cases are timedaccording to the time of FIGO surgical-pathologic staging made in1988: there are 19 cases inâ… stage, 25 cases inâ…¡stage, 3 cases inâ…¢stage. In the lesion 11 cases are limited to the endometrium group,9 cases are superficial myometrial invasion group, 17 cases are deepmyometrial invasion group, 10 of them took the operation ofhysterectomy only, others took the operation of hysterectomy(radical hysterectomy+double appendix exsection+ Pelvic kidneylymph node clearance method) 3 of them have lymph transferred. Allof above patients didn't take radiotherapy and chemotherapy beforeoperation. Take 11 cases of EIN, 10 cases of Endometrial hyperplasia,10 cases of normal endometrium samplex to compare with each other.All of the above cases are adopted by immunohistochemistry S-Ptechnique with monocolonal anti-E-CD-g Ab and anti-β-cat-g Ab.Refer the result to Schippir,take normal tissue as positivecomparison and classify according to the stain of cellular membrane,cytoplasm and nucleus. Statistics take Spss statistical package forwindows 12.0 to make x2 examination, Spearman analyzing method toanalyze the expression and relations between E-cad and β-cat indifferent types of endometrium, different surgical-pathologicstaging, histology grade, muscular layer invation and in lymph nodemetastic. Result: 1.The aberrant expression rate of E-cad transferred from normalendometrium to Endometrial hyperplasia-EIN-EndometrioidAdenocarcinoma rises gradually. The difference between them hasgreate statistics significance (P<0.05). 2.The aberrant expression rate of E-cad increases as the increaseof Endometrioid Adenocarcinoma pathologic graded. G1-G2G3(P<0.05).Inâ… .â…¡andâ…¢(p<0.05) muscular layer invasion and lymphnode transfer is higher than ones without (p<0.01), betweenâ… andâ…¡(p<0.01). 3.The difference of β-cat in normal endometrium, EIN andEndometrioid Adenocarcinoma has grate statistics significance.(P<0.01). 4.β-cat has great statistics significance in different clinicalStaging (P<0.05). Muscular layer invasion is higher than oneswithout (P<0.05). There is no apparent relation between differentdegree of histo-differentiation of β-cat EndometrioidAdenocarcinoma and lymph node transfer (P>0.05). 5.The expression of E-cad and β-cat in EndometrioidAdenocarcinoma is positive relation(R=0.758 P<0.01). Homo sapiens tipe E-cad gene maps at 16 pa22.1, when theexpression of epithelial cells E-cad weakened or vanished, it maylead to the grow of btumor cell, also easy to diastasis from primarylesion, transfer to sectional lymph or further. Decrease ofexpression for E-cad inactivation mechanism includes genemutations, transcription activity abnormality. The breakdown orassemble of E-cad and the redistribute of E-cad and fall off fromthe cell. The study discovers: normal expression rate of E-cad inendometrium is: normal Endometrium >Endometrial hyperplasia>EIN>Endometrioid Adenocarcinoma (P<0.05). This study indicates thatthe E-cad facilitates the progression of transformed endometriumtowards malignancy. Also discovers that, aberrant expression rateof E-cad increases (P<0.05). as the de-differentiation ofEndometrioid Adenocarcinoma and the increases of deepth of invasion.The aberrant expression rate of patiens with lymph transferrid ishigher than the ones without lymph transferrid (P<0.05). Therebyimplies that E-cad has relations with more aggressive malignantbehaviors of Endometrioid Adenocarcinoma and de-differentiation,aberrant expression of E-cad is one reason of tumor's obtainingof tall proliferation activity and invasiveness, lots of studiesconsider E-cad as the prognosi marker of EndometrioidAdenocarcinoma, the experiment verifies this point too. β-cat is cytoplasmic molecules composed of polypeptide chain.In normal cell, β-cat can join with E-cad, also be able to combinewith APC which contains (GSK-3β) APC and Axin. The main functionofβ-cat are as follows: â‘ Reciprocal effect with ES-cad celladhesion. â‘¡As one important member of wnt signaling pathways, itis related with the growth of embryonic development and tumor. Itcan transfer signals from cellular membrane to cytoplasm then intonucleus. Then take part in the WG/WNT signal which are composed ofAPC/β-cat/Tcf/lef/. Any abnormality during the process of β-catphosphorylations can lead to the increase of freeing β-cat:accumulation ofβ-cat's cytoplasm, nucleus are found in manytumors, however, there is vary little cell ieward accumulation innormal cell. The study analyzes the expression of β-cat in EndometrioidAdenocarcinoma and EIN,discovers that the aberrant expression rateof EIN and Endometrioid Adenocarcinoma group β-cat increasesgradually. As the increase of histological grade and carcinomatouschange muscular layer invasions is higher than ones withoutinvasions(p<0.05). The collection of β-cat in cytoplasm andnucleus can be seemed as important reference basis for EndometrioidAdenocarcinoma. The study indicates that the normal expressionrates for E-cad and β-cat is 4/37 (10.8%) in 37 cases ofEndometrioid Adenocarcinoma, however, tumors still occur.
