AIM: To observe the frequency, degree of the endodontic interappointment pain(EIP) during root canal therapy when prescribing NSAIDS prophylacticly and to search how EIP changes with time. Observing the correlation between the degree of the pain and the concentration of the PGE2 in periapical exudates during root canal therapy (RCT).METHOD: All of the cases we studied are from west china stomatologia hospital of Sichuan university processing root canal therapy. The first part is composed of two groups, one group includes 92 teeth defined routine therapy which employed routine RCT according to clinical protocol, the other group has 86 teeth prescribed with NAIDS (diclofenac sodium, yingtaiqing) after routine RCT. Statistic analysis the incidence, degree of pain at different timepoint after root canal therapy. The second part included 18 decay teeth after RCT, the periapical exudates were collected at immediate and 24h after root canal preparation to search the relationship between pain and PGE2. The concentration of PGE2 were assayed by radioimmunology kit. RESULT: The first part: l.The incidence of EIP in routine RCT group was 33%, while was 25.6% in the drug prescribed group, There's significant statistic variant (P<0.05). 2. The degree of pain in routine group was significant higher than drug group (P<0.05). 3. The incidence of EIP of both groups were dramatic high within ld> 2d^ 3d after RCT, then the pain relieved gradually, but incidence of EIP was higher in routine group at given time point. The second part: There's positive correlation between the degree of EIP and the level of PGE2, and the change of PGE2 concentration was intimately pertinent to the degree of lenitive pain. Last, we found that the concentration of PGE2 were variant significantly between patients who had no EIP and those had high level EIP(P<0.05).CONCLUSION: This study disclose that the concentration of PGE2 and change of pain after root canal therapy has a intimately positive correlation. During root canal therapy, prescribing yingtaiqing after root canal preparation for 3ds prophylacticly can reduce the incidence of EIP and level of severity. |
