| Objectives: The purpose of this experiment is mainly investigate therapeutic effect of Fleabane injection on the acute exacerbation stage of chronic cor pulmonale and approach Fleabane injection impact the plasma concentration of TNF-α and ET-1, so we presume the therapeutic mechanism of Fleabane injection to chronic cor pulmonale.Materials and Methods: Forty hospitalized patients which had been diagnosed the acute exacerbation stage of chronic cor pulmonale were recruited into the trials in Department of Respiratory in Huaxi Hospital from May 2004 to February 2005. We divided them into an experiment group (EG) and a control group (CG) at random . Each is 20 cases . There are 17 male and 3 female in EG, the average age is 69.0±6.8 years old, and there are 16 male and 4 female in CG the average age is 70.0±8.4 years old, the ages, gender, condition of the two groups are no statistics, their ages are limited 50 to 80 years old. At the same time we selected the healthy persons as a normal control group (NCG). There are 10 male and 5 female in NCG, the average age is 61.8±6.3 years old. they were treated by anti-infection, aspiration oxygen of conventional therapy. EG were treated by Fleabaneinjection on the base of conventional therapy every day. The course oftreatment is fourteen days.Results:1. The comparison of therapeutic effect of EG and CGThe excellence rate of EG is obviously higher than that of CG . there is statistical significance (P<0.05).2. Variation of EG and CG's SPAPEG and CG'SPAP are all obviously increased before treatment. There is no statistic difference in the two groups (P>0.05). The SPAP of the two groups after treatment were all decrease, but the SPAP of EG was significantly lower than that of CG (P<0.05).3. Variation of the plasma concentration of TNF-a and ET-1 between EG and CGThe plasma concentration of TNF-a and ET-1 in EG and CG before treatment are all obviously higher than NCG (PO.05). Although the plasm concentration of TNF-a and ET-1 in both groups after treatment were all decrease, they are obviously higher than NCG (PO.05). The plasma concentration of TNF-a and ET-1 in EG were significantly lower than CG (PO.05) after treatment.4. The analysis of correlation between TNF-a , ET-1 and arterial blood gas' indexThere were significant negative correlation between the plasma concentration of TNF-a of the both groups before treatment and PaO2 (r= -0.7272 , PO.05), there were no correlation between the plasma concentration of TNF-a of the both groups before treatment and PaCCh (r=0.5509 , P>0.05). There were no correlation between the plasma concentration of ET-1 of the both groups before treatment and PaC?2 (r= -0.168 , P>0.05). There were no correlation between the plasma concentration of ET-1 of the both groups before treatment and PaCC>2 (r= -0.0518, P>0.05). 5. Adverse effectThere were no obviously adverse effects in the course of treatment. Conclusions: Fleabane injection has obviously therapeutic effect on the acute exacerbation stage of chronic cor pulmonale and can lower PAP. the therapeutic mechanism of Fleabane injection on chronic cor pulmonale is that it can lower PAP and the plasma concentration of TNF-or and ET-1. We didn't find out the advers effects of Fleabane injection in the course of treatment. |
