| Purpose: To analyze and study the clinical characteristics of congenital superior oblique palsies and principal for the choice of surgical treatments; Methods: The clinical data surgical results from 87 patients with congenital superior oblique palsies (SOP) hospitalized between March 2003 to March 2005 were retrospectively analyzed. The simple SOPs were surgically treated by weakening the antagonistic muscles and yoke muscles. Other types of strabismus accompanied with SOP were treated at same time or separately with related surgical techniques according to the clinical features and examination results. Results: 64 of 74 cases with simple SOPs were successfully corrected by one time surgeries with success rate of 86.5%. 6 cases (8.1%) were found with relatively good results and 4 cases (5.4%) were failed. Successful results were obtained from 11 out of 13 cases with bilateral SOPs. The other 2 cases were diagnosed later as recessive bilateral SOPs and treated secondly by weakening the contralateral inferior oblique muscle with good results.
