| Objective : To investigate the relationship between serum leptin and prognosis in patients with acute myocardial infarction and to define risks factors ,so to improve therapy and prognosis and assess the severity of the condition better.Methods : 60 male patients who met AMI criteria were studied after they were hospitalized in ICU of Tianjin medical university general hospital from November 2003 to November 2004.We collected the clinical variables related with leptin,based on a signal factor logistic regression analysis to define the risk factors,then we performed the mulvarity logistic regression analysis and found independent risk factors,established logistic equation ,so we can define risk factors which have important effects on clinical prognosis.We also had a T-test on serum leptin level between the death nonsurvivor and the survivor and chi-square between different serum CKMB level and different infarction positions based on hyperleptinemia.also,to compared the true positive and the true negtive between serum leptin level and APACHE II score. Results: After a signal factor logistic regression analysis,we found thatAPACHE II score ,serum leptin level,glucose level,C-reactive protein,insulin level, IRI and serum CKMB level were predictors the prognosis of AMI. After the mulvarity logistic regression,only APACHE II score, serum leptin level,IRI and serum CKMB level were left .And seum leptin level is positive correlated with APACHE II score and IRI.There were significant differences between nonsurvivors and survivors on serum leptin level. There was difference among different CKMB levels.While there was no difference among different infarction position. The true negative of serum leptin to the prognosis of AMI is 52.6%;the true positive is 65.8%. The true negative of APACHE II score to the prognosis of AMI is 63.1%; the true positive is 70.7%.There are no significant different between serum leptin level and APACHE II score in judging the prognosis of AMI patients.Conclusion: There is hyperleptinemia in AMI patients .There are significant differences on serum leptin level between the survivor and the nonsurvivor . There are significant differences among different serum CKMB level on the numbers of hyperleptinemia.however,the different infarction positions are not.APACHE II score, serum leptin level,IRI and serum CKMB level are the influential factors on the prognosis in AMI patients.
