Objective Utilizing the disease risk prediction model evaluates the Return-on-Investment (ROI) of the type 2 diabetes risk factor management program, in order to prove it can make financial benefits while controlling the population type 2 diabetes risk. Methods A total number of 7600 people's health information, who had been not diabetic patients, were collected by the questionnaire and evaluated by the type 2 diabetes risk prediction model. And then all people were categorized into three groups (low risk, moderate risk and high risk) according to the quantity of risk factor. The health behavior guidance is offered to all people. And after 1 year, 7600 people's health information were collected and evaluated again. Finally, through comparing two evaluation results and basing on the annual medical costs for treating type 2 diabetes, the program's ROI was calculated. Results The absolute risk of total people dropped from 86.2697 to 72.5747. The program's ROI is 1.79:1 and the ROI of high risk group is 5.18:1. Conclusion In a short time, the type 2 risk factor management programs can make positive ROI while reducing the disease risk.
