| objective : To investigate the best time and improving of the method of cutting off the basis of the delayed skin flap .To provide the experimental basis for clinical transplantation of the skin flap .Method: Three white mini-pigs were chosed for experimental model .12arbitravy skin flaps were desighed and divided into two groups ,the experimental groups <6>and contrary groups <6>.From the second day after operation ,the perfusion of the microcirculation was inspected constantly .Untill the fifth day ,the skin flaps were given the training of cutting off basis .The traditional method of separation with rubber band was adopted for separation the tissue .The experimental group was ligated and obstructed by stages with operative suture of prefabricated basis part .Then we inspected the changes of the flow of microcirculation (pu),the MDA and tissular pathology.Result: After the formation of the arbitrary skin flaps ,the flow of the micro-circulation was increasing as time went on .After 2-3days ,the flow microcirculation obviously increased, recovering to the pre-operation level at the thirdday. Then the flow of microcirculation appeared plateau period, After 5 days the flow of microcirculation was slightly more than before operation . But from the third day to the fifth day ,the increasing of the flow of microcirculation was not obvious(p)0.01.The groups were given training of breaking off basis .after 5 days ,until the seventh day, the basis was cut off .in the experimental group ,the changes of flow of microcirculation was smooth ,the MDA was low ,the pu was in high level after cutting off the basis .On the contrary ,In the contrarygroup ,the change of pu was severe .The MDA was obviously higher than the experimental group, but the pu was in low level after cutting off basis .Conclusion: The earliest time of cutting off the basis was after plateau .We cut off the blood supply with the method of suture of prefabricated base part at the fifth phase ,making the transplantation of flaps "smoothly landing" Then the blood circulation of skin flaps can dynamicly .transit to the later stage of cutting off basis . |
