The Effects Of Rosiglitazone And Metformin On Clinical, Endocritic And Metabolic Features In Women With Polycystic Ovary Syndrome And Insulin Resistance

Objective: To compare the effects of rosiglitazone and metformin on insulin resistance (IR), androgensis and leptin level in women with polycystic ovary syndrome (PCOS).Method: Forty women with PCOS and insulin resistance were divided into two groups by random and treated with rosiglitazone or metformin respectively for three months, to observe the change of sex hormone, insulin sensibility and leptin level after treatment.Result: After treatment fasting insulin decreased greatly in two groups, In group treated with rosiglitazone reduced from 21.62±12.08mIu/l to 9.54±2.68mlu/l, p<0.01, and in another group reduced from 18.26±6.22mlu/l to 13.03±3.47mIu/l, p<0.01, significant difference was found between two groups. Insulin sensitivity index (ISI) increased significantly in two groups. Serum level of LH, LH/FSH ratio, testosterone, androstenedione, dehydroepiandrosterone sulfate and leptin level also decreased significantly after administering medicines. But the differences of two groups were only found in the level of testosterone, androstenedione and leptin. After three months clinic symptom improved, the menstrual cycle shortened and ovulation resumed spontaneously.Conclusion: Insulin resistance is related to leptin level and androgen level in women with PCOS and IR. Insulin sensitizer can reduce androgen coming from ovarian and adrenal gland and improve leptin resistance by decreasing insulin level and increasing insulin sensitivity to peripheric tissue. Rosiglitazone more significantly reduce insulin level and improve metabolize syndrome than metformin.
