Objective: the purpose of this study was to assessed that the concentrations of interleukin-1alpha (IL-la), interleukin-lbeta (IL-1β) and interferon-gamma (IFN-y) in the supernatant of peritoneal fluid macrophage and peripheral blood monocyte of endometriosis patients and to explore the role of macrophage and monocyte in the pathogenesis of endometriosis and their effect on the development and progression of endometriosis.Methods: Forth women with endometriosis who were diagnosed by histopathologist after operation at stage II -IV according to the criteria by Revised American Fertility Society were selected as endometriosis group, and eutopic endometrial samples were histologically divided into proliferative (n = 25) and secretory (n = 15) phases.Thirty women with leiomyoma or benign ovarian tumor were selected as control group.Sixteen women with leiomyoma and eutopic endometrial samples were histologically divided into proliferative (n = 11) and secretory (n = 5) phases. Fourteen women with benign ovarian tumor and eutopic endometrial samples were histologically divided into proliferative (n = 7) and secretory (n = 7) phases. Isolated and cultured their peritoneal fluid macrophage and peripheral blood monocyte respectively, the concentrations of IL-lα, IL-1β and IFN-γin culture media were determined by enzyme-linked inununosorbent assay(ELISA).Result: The mean IL-1α concentration in peritoneal fluid macrophage supernatant of endometriosis group is (110.2±34.2) pg/ml ,siginificant higher than (88.2±34.6) pg/ml of control group (P<0.01). The mean IL-ip concentration in peritoneal fluid macrophage supernatant of endometriosis group is(90.3±14.6) pg/ml ,siginificant higher than (78.3±15.5) pg/ml of control group (P<0.01).The IL-lou IL-ip> IFN-y concentration secreted from peritoneal fluid macrophage and peripheral blood monocyte superaatanst have no significant difference in proliferative phase and secretory phase(P>0.05) in endometnosis group same as in controls group. The IL-la concentrations in peripheral blood monocyte supernatant of endometriosis and control groups were (107.3±34.4) pg/ml, (83.2±35.8) pg/ml respectively, and has statistic significance between groups(P<0.01). The IL-^concentrations in peripheral blood monocyte supernatant of endometriosis and control groups were (90.5±23.9) pg/ml >(74.7±23.5) pg/ml respectively, and same as IL-la's statistic (PO.01). The IL-la> IL-ip, IFN-y concentrations in peritoneal fluid macrophage and peripheral blood monocyte supematanst of endometriosis were no significant difference in II -. ni> IV stage.(P>0.05),and that is irrespective with clinical stages. Peritoneal fluid macrophage secretes much-more IL-la > IL-lpthan peripheral blood monocyte in both groups (P<0.01). Both the level and the positive rate of IFN-ysecretion by peritoneal fluid macrophage and peripheral blood monocyte were low in these 2 groups, and no significant difference was shown(P>0.05).Conclusion: The ability of endometriosis peritoneal fluid macrophage secreting IL-1 was obviously enhanced, but the secretion of IFN—ywas low. Increasing the secretion of IL-lcu IL-ipinduces the changing of immune system in abdominal cavity and Total body.The release of cytokines may enhance the capability of endometrial cells to implant and grow into the peritoneal host tissue.This may play an important role in the physiology of the human endometrium and the pathophysiology of endometriosis by its role in cell growth,angiognesis and facilitate the occurrence and progress of endometriosis.More and more often scientific studies reveal the key role ofimmunological disorders as the factor contributing to the development of endometriosis.The impairment of local cytokine balance in women with external endometriosis has a pathophysiological role in the disease such as IL-1 ,IFN—y.
