Objective: To observe the clinical therapeutic effect of the Chinese medicine paved on the umbilicus and enema in the treatment of refractory cirrhosis ascites with azotemia. To observe the content of NO, ET and endotoxin in blood serum .Methods: 60 cases were selected to take part in the randomized, placebo-controlled , double-mind trial. 60 patients were randomly divided into a treatment group and a control group. 30 cases of the treatment group were treated with basal therapy and the Chinese medicine poved on the umbilicus and enema, while 30 cases of the control group were treated with basal therap, placebo-medicine paved on the umbilicus and enema. Their total therapeutic effects , improvement of ascites, content of NO, ET and endotoxin in blood serum were observed and compared between two groups.Results: There was significant improvement in abdomen distention and oliguria after the treatment of the treatment group. The total effective rate of the treatment group and the control group was 86.67%, 46.67 % respectively. There was significant difference between the treatment group and the control group (P <0.01). There was significant difference in PVD, QVP , SVDand QSV before and after the treatment in the treatment group. There were no significant difference in PVD, QVP , SVDand QSVbefore and after the treatment in the control group. The hemodynamics in the control group was not significantly improved. The content of NO, ET and endotoxin in blood serum decreased significantly after the treatment in the treatment group. Live and kidney function improved in both the treatment group and the control group. ALT, AST and TB decreased while ALB increased. There was no significant difference of the kidney function in the control group while significant difference before and after the treatment in the treatment group.ConclusionsrThe Chinese medicine paved on the umbilicus and enema has obvious effect on improving the hemodynamics of refractory cirrhosis ascites with azotemia. The content of NO, ET and endotoxin obviously decreased after the treatment. Ascites and symptoms of abdomen distention and oliguria were improved after the treatment. The Chinese medicine paved on the umbilicus and enema has definite clinical therapeutics effect on the treatment of refractory cirrhosis ascites with azotemia. Side and toxic reaction were not seen in the clinical cases.
