Relative Research Of Blood Serum β-HCG Quantity, E₂, P And E₂/P In Fallopian Pregnancy, Unstable Fetal Movement And Fetal Leakage

[Objective] To study the relativity of blood serum β- HCG quantity , E₂, P and E₂/P in fallopian pregnancy, unstable fetal movement and fetal leakage. Andapproach the objective evidence of the above-mentioned pregnancy disease of TCM differential diagnosis and differentiation of symptoms and signs which is good to the diagnosis, treatment and prognosis of disease.[Method] To assemble hospital patients who is been visiting the first affliated hospital of Guang Zhou TCM university from Jan 2004 to Apr 2006 for the disease of ectopic pregnancy(vital-qi difficiency and blood stasis type 34 cases, fetus obstructing collaterals 16 cases), unstable fetal movement(renal deficiency 18 cases)and fetal leakage(renal deficiency 40 cases). Then they are divided into four groups based on the three entities and four patterns and use the retrospective case-control study method, analyze the difference between blood serum β - HCG quantity, E₂, P and E₂/P in the types mentioned above. Carry out statistical analysis with SPSS10.0 package and compare them with independent T-test.[Result] There is no significant difference in age and days of suppressed menstruation between groups (P>0. 05) . The value of blood serum P - HCG quantity and E₂, P in tubal pregnancy group is lower than that of threatened abortion group, which shows great difference between them (P<0. 01) but no difference in E₂/P (P>0.05).In fallopian pregnancy group:P value in vital-qi difficiency and blood stasis type is lower than that of fetus obstructing collaterals which shows great difference (P<0. 01. While E₂/P is higher comparing the first type to the second one (P<0. 05) .If E₂/P≥70, vital-qi difficiency and blood stasis type occupies 29.4% (10/34 cases), fetus obstructing collaterals 0 case;If E₂/P^45, vital-qi difficiency and blood stasis type occupies 41.2% (14/34 cases), fetus obstructing collaterals 12.5% (2/16 cases);If E2/P^ 17, vital-qi difficiency and blood stasis type occupies 76.5% (26/34 cases, fetus obstructing collaterals 56. 3%(9/16 cases). While the difference between blood serum fi- HCG quantity and E2 is not significant.P value in unstable fetal movement type (renal deficiency) is lower thanthat of fetal leakage(renal deficiency) which suggests great difference(P<0. 05) . As to E2/P, it is vice versa. There is also great difference (P<0. 05)between two groups. And there is no significant difference between blood serum3- HCG quantity and E2.[Conclusion] 1. In the process of observation on diagnosis and treatment about fallopian pregnancy, unstable fetal movement and fetal leakage, except that blood serum P- HCG quantity and P are directly related to pregnancy development, E2 and E2/P also possess practical meanings. Therefore the value of E2, P and E2/P should be emphasized.2. The level of the three hormones ( ï¿¡ - HCG, E2, P) obviously reduces in fallopian pregnancy which suggests that the discrimination in tubal pregnancy and threatened abortion, three hormone level - testing is helpful for early assisting diagnosis.3. The test of hormone level in ectopic pregnancy provides objective laboratory evidence for the syndrome of vital-qi difficiency and blood stasis type and fetus obstructing collaterals. When the patients got the disease of fallopian pregnancy , P value is low enough to result in the elevation of E2/P proportionality , then P value maybe not enough to support pregnancy which easily leads to fallopian tube pregnancy abortion. If E2 stays in a high value , that will lead to high proportionality of E2/P which is easier to result in the rupture of ectopic pregnancy. All metioned-aboved suggests that as to those patients whose E2/P is obviously rising , ectopic pregnancy rupture and abortion should be paid attention to in the medicine expectant treatment of ectopic pregnancy.4. There is obvious difference in the P level of unstable fetal movement and fetal leakage. Fetal leakage is a slight clinical symptom in the process of threatened abortion,while unstable fetal movement is a kind of serious clinical stage. The proportionality of Pand E2/P between unstable fetal movement and fetal leakage provides their objective evidence for thedifferential diagnosis of diseases.
