| objective :to investigate the significance of the change to urine RBP level in the early renal tubular injury of newborn asphyxia and the change tendencies of the urine RBP level in the different groups in different day-ages and to analyze the possible factors to influence the recovery of the renal tubular function after asphyxia in neonate.Methods①132 cases of full-term neonates with asphyxia (mild-degree 67,severe-degree 65)and 48 cases of normal ones(the control group)were measured the content of RBP with ELISA on day 1,3,7,and 10,α1-microglobulin(α1-MG) with competitive radio-immunity of urine only on day 1.②Case-control study was adopted and132 asphyxiated neonates were divided into the abnormal group(63 cases with the abnormal urine RBP level on seventh day after birth ) and the control group(69 cases with the normal urine RBP level). Design the questionnaire and obtain the data concerned with gravidity,accouchement of mother and newborns, and to carry out the Logistic regression analysis(SPSS11.5) .Result①The urinary RBP level andα1-MG level of asphyxiated newborns were higher than those of the normal control group on days 1,and the difference between the two groups is significant in statistics (P<0.001).②There were significance of difference between two methods by chi-square test(χ2=59.73,P<0.001),hypothesis test of Kappa had statistical significance, ( P<0.001) and k=0.67<0.75,which indicated agreement.validity of the two methods were moderately.③Analysis of variance (ANOVA) of repeated measurement data indicated there were significance of difference in urine RBP level among the three groups(P<0.01). Further, comparison in Student-Newman-Keuls test indicated the difference between the optional two group had statistical significance( P<0.05)④The changing tendencies of the urine RBP level in different groups were different in different day-ages by ANOVA(P<0.01).The urine RBP level was gradually decreased with the increasing of day-ages in normal full-term neonates in 10 days and in asphyxiated newborns it reached the highest level on day 3 after birth,and then gradually decreased.⑤There were significant difference in the levels of urine RBP in three groups by ANOVA on the seventh day and on the tenth day after birth(P<0.01).Further, comparison in... |
