Objectives: To study the relationship between the Chinese medicine syndromes of MS and IR , TNF-α ,and then explore the pathogenesis of MS,which provide the experimental indexes for the Chinese medicine diagnosis of MS in objective and dialectical way and the theoretical basis of integrated traditional and western medicine treatment.Methods: According to IDF diagnosis criteria and TCM dialectic criteria, We selected 80 cases(42 male and 38 female) and divided them into four Chinese medicine syndrome groups which include phlegm-dampness accumulation syndrome (PDAS,23) , phlegm accumulation and blood stasis syndrome (PABSS,21) , Yin deficiency and heat excess syndrome (YDHES,17) , Qi and Yin deficiency syndrome (QYDS,19). 20 healthy people (11 male and 9 female) were taken as control group.(1) We take their blood pressure and waistline on an empty stomach.(2) We detected fasting plasma glucose(FPG), fasting insulin (FIns) , triglyceride (TG) and high density lipoprotein-cholesterol (HDL-C), Tumor necrosis factor- α (TNF- α )through taking blood suction from the Vein.(3) The reverse of the Product of FPG and FIns is used as the insulin sensitivity index (ISI) : (ISI)=-ln(FPG×FIns).Results: Compared with the control group, the ISI of MS group was significantly lower(P<0.05).The ISI of the four Chinese medicine syndrome groups were lower than that of the control group significantly (P<0.01), the degree of ISI lowing level was QYDS < YDHES < PABSS < PDAS. The differences between the two groups was significant ( P<0.01). ISI is negative related to TNF-α level (P<0.05). Compared with the control group, the TNF-α level of MSgroup was significantly higher (P<0.05).The TNF-a level of the four Chinese medicine syndrome groups were higher than that of the control group significantly (P<0.01), the heightening level was QYDS >YDHES>PABSS>PDAS. The differences between the two groups was significant ( P<0.01).Conclusions: 1. IR is closely associated with MS. Cytokines, especially TNF-a played a very important role during IR path course.2. The common Chinese medicine syndromes show different phases in MS physiopathologic course. The difference of IR degree hints that we can take ISI as one of the reference indexes of Chinese medicine dialectical typing in MS.3. Phlegm and stagnant blood are etiological factors and pathologic results of IR.4. TNF-a can be taken as one of the sensitive indexes to show IR degree in MS.
