Objective The aim is to investigate the fundamental mechanisms on the origin, growth and rupture of intracranial saccular aneurysms by the pathological methods. Methods 18 cases of intracranial saccular aneurysms were investigated. H&E staining was used for microscopic analysis, and these cases were stained by the specific monoclonal antibodies for T lymphocyte(CD3), B lymphocyte (CD20), monocyte/ macrophage(CD68) respectively. Simultaneously, ultrastructural study was performed by the transmission electron microscope. Results The normal three-layer structure had to lose in all intracranial saccular aneurysm walls, the fundamental histopathological changes were injury, denaturalization and repair. Under the light microscopy observation, the intracranial saccular aneurysm specimens showed structure changes, including injury of endothelial cell, disrupture or disappearance of the internal elastic layer, scarce of contractile fiber cells. The regional necrosis and different stage of atherosclerosis could be detected in aneurysms walls cells. Thrombosis and different degrees of organization were found in most of the cases, fully formed organization is rare. Specific monoclonal antibodies staining show that the infiltrations of immune cells were obvious which was consisted of T lymphocyte, monocyte/macrophage and no B lymphocyte.
