Objective: To explore effect of Shenfu injection ( SFI ) the dose and time-effect relationship Shenfu injection ( SFI ) on myocardium protection in mitral valve replacement ( MVR ) druing cardiopulmonary bypass ( CPB ) so as to support further theory evidence for clinical prper medcation.Methods: 40 patients ( ASAⅡ~Ⅲ, NYHAⅡ~Ⅲ, 38~50 years ) undergoing elective MVR were randomized into control group ( group C ) and trial groupsⅠ,ⅡandⅢ. Patients in group C accepted normal saline during whole CPB. Trial groupsⅠ,ⅡandⅢwere respectively put into with SFI 1.0 ml/kg, 1.5ml/kg and 2ml/kg at the time of routine primiing fluiding as well as after opening aorta. MAP was monitored before CPB ( t1 ), at times of initiating anterior parallel circulation ( APC ) ( t2 ), of posterior parallel circulation ( PPC ) ( t3 ) and CPB stopping ( t4 ). Other henmodynamics markers ( HR, rate of spontaneous heart beat and cardiac rhythm ) were recorded after restoration of heart beat. Blood samples were taken from central vein to determine the serum concents of cardiac neuropeptide Y ( NPY ) , cardiac troponin I ( cTnI ) and the activity of glutathione peroxidase ( GSH-PX ) in each group respectively before the operation ( T0), at the time of termination of operation ( T1) and 3 hours ( T2 ), 24 hours ( T3 ) after operations.
