Objective: To explore the role and relationship of thrombomodulin and plasminogen activator inhibitor-1 in the progression of diabetic retinopathy, and to provide a new theoretical foundation to diagnose early and therapy effectively diabetic retinopathy.Methods: Forty eight Wistar rats were divided randomly into two groups. There were twenty four in normal control group(NC) and twenty four in diabetes mellitus group(DM). The subjects in diabetes mellitus group were injected intraperitoneally with streptozotocin(STZ,50mg/kg) while the ones in normal control group were injected intraperitoneally with the similar dose of citric acid buffer. At six weeks, ten weeks, fourteen weeks after injection, there were eight rats in both groups respectively killed.The blood sample were obtained to measure the levels of FINS, C-peptide, TM and PAI-1.The morphological changes of rat retinas were observed under light microscope and electric microscope and analyzed by an electronic computer image processing analytical system.Results: There were a significant difference in the levels of fast blood glucose(FBG) between NC and DM at every stages(P<0.01), the levels of FBG of DM were higher than that of NC. The levels of FINS of C-peptide were lower than that of NC at every stages(P<0.01). The levels of TM and PAI-1 of DM were higher than that of NC at every stages (P<0.05,P<0.01,P<0.01,respectively). Plasma TM was in positive correlation with PAI-1 in DM(R=0.786, P<0.01). With the development of DM ,membrane disks spaces in rods of DM were enlarged gradually,dividing and dissolving locally.The capillaries basement membrane increased gradually, and capillaries became narrow and even occluded.The mitochondria swelled and even denatured.Conclusion: The levels of plasma TM and PAI-1 in DM group increased gradually as retinopathy progressed. Plasma TM has positive correlation to PAI-1 in DR. According these we can conclude that the plasma TM and PAI-1 might be involved in the pathogenesis of DR. The plasma TM and PAI-1 are important to diagnose early and therapy early diabetic retinopathy.
