OBJECTIVE:â‘ To explore the distribution of Toll-Like Receptor 3 in term human placenta of normal and HBsAg-positive pregnant women.â‘¡To explore HBsAg-positive pregnant women'placentas HBV infection state and risk factors.â‘¢To explore the role of TLR3 in placenta HBV infection.â‘£To study the risk factors of HBV intrauterine infection.METHODS: 473 pregnant women and 473 newborns were collected between July, 2001 and September, 2005 in Taiyuan infectious hospital.The epidemiological base line data involving gestation and postpartum were also collected. Newborns'femoral vein blood within 24 hours after birth and maternal elbow vein blood were collected. At the same time, the placental tissues were also collected. Following studies were performed: HBsAg, HBeAg in gravidas'and newborns'serum were tested by ELISA; HBV DNA in both serums were detected by Fluorescence Quantitative PCR (FQ-PCR); HBsAg and TLR3 in placentas were detected by immunohistochemistry ABC; Both HBsAg and TLR3 in infected placental tissues were detected by double labeling immunofluorescent histochemistry assay; risk factors of HBV intrauterine infection and placental HBV infection were analyzed by nested case-control study. Furthermore, the interactions among risk factors were also analyzed. All of the data was analyzed by the statistical packages Visual Fox Por 6.0, SPSS 11.5 for windows.RESULTS: 1. The positive rate of TLR3 in HBsAg-positive pregnant women was 74.51(76/102). TLR3 was mostly located in the cytoplasm, a part on the cell membrane and occasionally in the nuceus. TLR3 positive rate gradually increased from the maternal side to the fetal side (trend test P<0.001) in the placental cell layers. Maternal decidual cells (30.39%) trophoblastic cells (29.39%)> villous mesenchymal cells (20.93%)> villous capillary endothelial cells (9.51%).3.
