OBJECT: To investigate the relation between Von Willebrand factor(vWF) and the ambulatory blood pressure monitoring(ABPM) parameters or carotid artery intima-media thickness(IMT) in aged patients with essential hypertension.METHODS: The ambulatory blood pressure monitoring, carotid artery intima-media thickness and Von Willebrand factor are measured in 60 patients with essential hypertension and 30 controls. According to the IMT of carotid artery, we classified hypertension patients as IMT increased group(n=25) and no IMT increased group(n=35). The control group, no IMT increased group and IMT increased group are compared in ABPM parameters and vWF level. The correlations between vWF and ABPM parameters or IMT were analyzed by statistical method. RESULT:1. There were no significant differences in clinical manifestation and biochemical parameters between the hypertensive group and control group(P>0.05). The parameters of ABPM, vWF level and IMT of carotid arteries in hypertension group were higher than those in control group(P<0.01).2. There was a significant correlation between serum level of vWF and the IMT of carotid arteries. The correlation coefficient is 0.385(P<0.01).3. There were no significant differences in age,sex,body mass index and other serum makers(TC,TG,LDL-C,HDL-C) in the control group, no IMT increased group and IMT increased group.But there is a increasing trend in some ABPM parameters(24h mean systolic blood pressure,pulse pressure and the non-dippe rhythm) and vWF level(P<0.05). In addition, the vWF level of no IMT increased group was higher than that of control group.4. The vWF level of aged patients with essential hypertension was associated with pulse pressure(PP) and blood pressure circadian rhythm. Standardized coefficient of regression is 0.301 and 0.445(P<0.01).CONCLUSION:1. There are endothelium dysfunction and prothrombotic state in the aged patients with early essential hypertension.
