Clinical Studies Of Early Enteral Nutrition In Prophylaxis Of Stress Ulcer After Severe Head Injury

Objective:Stress ulcer, a most frequent visceral complication following severe head injury, which can cause deadly stress ulcer bleeding and enterogenic infection, has been a potential threat against critically ill patients. Meanwhile, the gastrointestinal tract acts as a important role in the systemic inflammatory response syndrome after severe trauma. That is to say, it is not only the target organ, but also the trigger. During the course of stress ulcer developing, many cytokines have been brought into full play, thus a net formed. The contents'changing of related cytokines response the state of gastrointestinal tract and whole body minutely. On prevention and cure project of stress ulcer, most investigations focus on drug and seldom have been explored about early enteral nutrition. Especially, prospective randomized controlled trials are still destitute in clinic. A few retrospective studies mostly depend on clinical observation. For drug application can not yet cut off stress ulcer ideally, it shows great sense that whether early enteral nutrition could protect mucosa effectively and serve as a major measure for stress ulcer prophylaxis and treatment in the early afte aevere head injury. This study, therefore, is aiming to investigate the effect of early enteral nutrition on preventing stress ulcer and reducing the intensity of stress reaction by surveying the contens changes of related cytokines, and try to afford a support so as to practice early enteral nutrition in clinic scientifically.Methods:1.A total of twenty-six cases of severe head injury patients were randomly divided into TPN group and EEN group averagely. Each case accorded with the study request. And the normal control group was set too. Patients of TPN group were given early total parenteral nutrition, while patients of EEN group were given early enteral nutrition and some essential parenteral nutrition.
