| Objective:Rheumatic heart disease (RHD) is a serious problem which threatens human health throughout the world, incidence in China was quite high , the valve which is commonly involved in RHD is mitral leaflets . Mitral valve replacement (MVR)is an effective method in treating mitral valve disease , but left ventricular performance is shown to deteriorate to varying degree in some patients after MVR. The reason which reduction of heart function occurs is not only due to abnormal hemodynamic but also due to chronic rheumatic cardiomyopathies and intramyocardial arteriolar wall thickening leading to obliteration of the lumen . Left ventricular function and myocardial fibrosis before the MVR have a close correlation with intra-operative , post-operative motality and survival probability , so assessing myocardial function correctly pre-operative is the key to successful MVR. The methods of traditional echocardiography which assess left ventricular function can be influenced by preload and afterload , accordingly this method can't measure the heart function properly. Recently Quantitative Tissue Velocity Imaging(QTVI) is a new technique which can detect shift signal directly from myocardium, and can measure velocity, time and displacement at different segments of ventricular wall.
