OBJECTIVES: This study is to investigate the effects and significance of Spironolactone -an aldosterone receptor blocker on some neurohormonal factors(PRA,AngII,ALD,and ANP),vascular endothelium function and heart function in chronic heart failure patients with NYHA class II -IV.METHODS: The Patients(n=148) with systolic heart failure with New York Heart Association (NYHA) class II to IV were randomly divided into two groups: spironolactone group (n=78) given spironolactone 40mg per day and control group (n=71),both two groups were on the base heart failure therapy with diuretics,digoxin,and ACE inhibitors. After 6 months'therapy, PRA,AngII,ALD,endothelium dependent blood tube bore dilatation LVEF,NYHA class were measured. The patients were monitored either by telephone or by correspondence every 3 months,the end point was cardiac death or noncardiac death.RESULTS: In either the patients with NYHA class IV or the patients with NYHA class II and III: The PRA,AngII,ALD,ANP in spironolactone group were lower at 6 months after treatment than those before treatment and those of the control; Endothelium dependent blood tube...
