Objective: To study the impact of hepatitis B virus (HBV) genotype on the development of HBV related hepatocellular carcinoma (HCC).Methods: 124 HCC patients and 124 case-controled chronic HBV carriers were involved in the study. HBV genotypes were determined by restriction fragment length polymorphism analysis or sequencing. Clinical data of HCC patients infected by different HBV genotypes were also compared. X gene (HBx), enhancer II (Enh2) and basic core promoter (Bcp) of HBV in sera from HCC patients were amplified by nest PCR , and sequenced and then compared with the sequences in Genbank. Logistic regression analyses was used to identify the variables of hepatitis B e antigen (HBeAg) and the amino acid changes in HBx,Enh2 and Bcp impacting on the relationship between HBV genotypes with HCC.Result: 1 .The genotypes of HBV in all patients are genotype B or C. Genotype B and genotype C in chronic HBV carriers and HCC patients were of 62.21% and 37.9% , vs. of 41.5% and 58.5%, respectively. 2.The average age of HCC patients infected by genotype B was about 50 years old similar to that in genotype C. The HBV genotype in HCC patients younger than 35 were mainly genotype B, whereas genotype C appeared more often in the group from 36 years to 50 years,and there was no significant difference in the infected HBV genotypes in the group of HCC patients elder than 50. 3. Clinically, the serum total bilirubin level (56.3±79.5vs.23.7±18.3, P =0.015) and the rate of extrahepatic metastases of HCC (20.7%vs.0%, P=0.01) in HCC patients infected by HBV genotype B were higher than those in genotype C. 4. In the virology of hepatitis B, there were no difference in the level of HBV DNA between genotype B and C in HCC patients, however the positive rate of HBeAg in HCC patients infected by HBV genotype C were higher than those in genotype B...
