Background and ObjectiveBrachial plexus injury is constantly a tough problem in clinical traumatic surgery,especially total brachial plexus or lower trunk injury.At present,there isn't any effective restorative procedure to repair hand intrinsic muscles.In this study,we make use of the successful experience of reconstruction of opposition and adduction of halluces in severe hand damage on account of firearm wound,which utilized inosculating blood vessel,skin flap transplantation of musculus extensor hallucis brevis and musculus extensor digitorum brevis. It might be an operative therapia to utilize anastomosing blood vessel, skin flap transplantation of musculus extensor hallucis brevis and musculus extensor digitorum brevis to reconstructe lower trunk of brachial plexus injury, especially oboslete damage of hand intrinsic muscles. Nevertheless, because of the specificity of lower trunk of brachial plexus injury, it's hard to find any mobility shift nerve branch at wrist recipient site for splicing directly, which is located at upper arm or elbow (such as lower trunk of brachial plexus injury).
