Diffuse Interstitial Lung Disease-a Clinical Analysis Of 33 Cases

Objective: To improve the cognition for the diagnosis, therapy and management of diffuse pulmonary interstitial lung disease by investigating and summarizing its clinical features and therapeutic effects.Methods: Retrospective analysis was conducted on 33 cases of ILD according to the X-ray and chest CT examination, including the clinical features, chest X-ray, chest CT, lung function examinations, arterial blood gas analysis, other laboratory examinations, bronchoalveolar lavage, lung biopsy and the treatment effect. All the patients gives supporting treatment, 10 of them were treated with macrolides, 19 (73.1%)of them were treated with corticosteroids, 3 cases had the bronchoalveolar lavage under the anaesthesia.Results: all of the patients(male 18, female 15), aged 30～75 years with an average of 52.9±10.7 years(>50 years 18 cases) , 28 (84.8%) out of 33cases had an insidious onset of progressive dyspnea, 17(51.5%) had nonproductive cough, 23(69.7%) had Velcro rale and 6 (18.2%) had finger clubbing. All of the cases had the X-ray and the chest CT, exhibit diffuse nodular, glassy, reticular, stripe and alveolate lesious, and so on. All the patients had the lung function examination, 29 of them had restrictive ventilation dysfunction, 2 of them had the combined ventilation dysfunction, 2 had small airway dysfunction. Arterial blood gas analysis indicated 22 cases (66.7%) had hypoxemia, 9 had respiratory failure, 16 had hyperventilation, 1 had CO₂ retention. 19 cases had lung biopsy: 16 cases by the transbronchial Lung Biopsy (TBLB), 1 case by percutaneous CT guided biopsy of the lung; 2 cases by video assisted thoracoscopic, 1 case by open lung biopsy.
