| The attending physician in charge is a hospital management model commonly and successfully used abroad. Gradually applied in some domestic hospitals, this hospital management model has achieved remarkable results. How to apply the management model of the attending physician in charge in a general hospital, which is of huge size and with extensive specialties and arduous duty, is a major issue worth studying.Firstly, this study aims at establishing a new medical management model which can be used in general hospitals. On the basis of the separation of academic titles and appointments, we divide the attending area into several medical groups, which have attending physicians, competent physicians and rule by physicians. These positions are set by needs and managed by classifications, with each class directly responsible. Secondly, this study aims at the separation of academic titles and appointments for professional and technical positions. Physicians have to compete for the positions in attending medical groups by the basic qualities and procedures required, no matter what positions they used to hold. Thirdly, this study aims at choosing three indicators to evaluate the effect of the attending physician in charge. Indicators include: 1) quality of medical care (the number of discharged patients, the average length of stay, infection rates, complication rates, average hospital charges, and medicine ratio); 2) medical service satisfaction rate; and 3) human resources management (medical staff structure). Statistical analysis shows that the experimental group (attending physician in charge) is better than the control group.Therefore, the management model of the attending physician in charge can help to improve the quality and efficiency in general hospitals. It contributes to the promotion of hospital management, energy management and incentive management, as well as to the separation of academic titles and appointments and the management of sub-specialty, providing an effective management tool. It also contributes to the introduction of competition mechanism, full play at all levels of physicians and initiative to promote progress and innovation in medical technology. |
