Objective: The surgical operation is the main therapy of Rectal Cancer. After operation,Patcent may unadvoidably get urinary functional disturbance due to the pelvic nerve injury. Now TME is the gold standard of curing Rectal Cancer in the world. Based on TME, To analyze and evaluate the effects of pelvic autonomic nerve preservatio(nPANP)on curative excision of male patients with advanced rectal cancer. This study was undertaken to test the efficacy and feasibility of this resection of rectal cancer.Methods: From January 2002 to January 2003,Twenty-six male patients with rectal cancer were performed with Pelvic Autonomic Nerve Preservation and Total Mesorectal Excision (TME)in the Department of Colorectal and Anus Disease ,the Third Affiliated Hospital of jilin University. The controlled group (thirty-eight patients)were performed with TME .On the basis of TME principle, the operation segregates the paries posterior of rectum sharply. During the course of disconnecting rectum, the doctor should pay attention to the exactthe position of autonomic nerve; whether the tumor invades the nerves and metastasis, it determines whether it retains the nerves.The main preservation nerves have:①Superior hypogastric plexus nerve;②Hypogastric nerves;③Pelvic plexus;④Pelvic splanchnic nerves.The urinary and sexual functions and recurrence rate and 3 year survival rate of the experiment group and the controlled group were retrospectively analyzed by t test orΧ2 test or Fisher's test.Conclusions : The curative excision with PANP for rectal cancer can reduce urinary and sexual function disorder and doesn't increase the local recurrence(LR) rate and doesn't decrease the 3 year survival rate.
