Study On The Hospitalized Expenditure Among Patients With Breast Cancer And Its Influencing Factors

BackgroundAt present, the reformation of our country's medical safeguard system is in the crucial phase, its most effective operation is to control the medical expenditure rising. But in the recent years, our country's medical expense is growing by 30% speed every year, far surpassed GDP's rising rate, and it will inevitably affect the development of our country's economy and the enhancement of the people's living level. Therefore how to control the medical expenditure rapidly rising is become the question which people pay attention to and urgently to be solved. In the medical expenditure constitution, the hospitalized expenditure accounts for a larger proportion, simultaneously it is also the researchers most concerned. If we want to effectively control the medical expenditure rising, especially the hospitalized expenditure rising, at first, we must be clear about the influencing factors of the hospitalized expenditure. Presently, medical expenditure management organizational reformation is unceasingly thorough at each place, the statistical analysis of disease medical expenditure may further deepen the reformation of medical expenditure system, and it can also provide the valuable reference data. We should use the science methods to research the hospitalized expenditure has the important practical value, which has the important practical value to effectively use the health resources, display its biggest medical benefit and guarantee the medical service quality, and control the growth of the medical expenditure.Objectives of the researchThis article used the breast cancer's patients of the Shan Dong University & Qi Lu hospital from 2001-2006, we took a systematic analysis about the data. Through the time series contrast analysis of breast cancer's patients about the hospitalized expenditure for 6 years, as well as the regression analysis of influencing factors, and to carry on sorting the relevant size of the factors affected. So we can grasp the distribution and the constitution of the hospitalized expenditure, and the influencing factors. All these can help the hospital policy-makers to formulate the expense control measures, and better control the hospitalized expenditure, to provide the positive reference, and so on.MethodsThe investigation chose the Shan Dong University & Qi Lu hospital, and used the material of the patients and their hospitalized expenditure from 2001-2006 about each year, who was diagnosed for breast cancer and had left the hospital in hospital medical record. Firstly, according to the sixth edited publication of the "Internal medicine" by the People's Medical Publishing House, which is the national higher medicine colleges and universities' teaching material, we integrated the case of illness meeting with the breast cancer diagnosis and the standard basis, rejected the hospital case of illness which didn't conform to the research conditions. The investigation obtains the effective female breast cancer's patients 534 altogether, and we carry on the analysis of its hospitalized expenditure constitution, the analysis along with the time changing tendency, as well as the influencing factors of the hospitalized expenditure. The statistical methods of this investigation contains the descriptive analysis, the regression analysis of many influencing factors and the time series contrast analysis. The study used the SPSS13.0 statistical software package, to carry on the statistical analysis.Main resultsThe research result mainly includes: (1) generally speaking, the charges for medicineaccount for the biggest proportion in the hospitalized expenditure of the patients, about34.3%~49.5%.(2)The patients who have public health services and the medical insurance cost a higher hospitalized expenditure. (3)The result of the hospitalized expenditure and the hospital days number relevant examination showed that, the hospitalized expenditure and the hospital days' Pearson correlation coefficient is 0.782, p<0.001, so there exists linear correlation relations between the hospitalized expenditure and the hospital days. (4)The regression analysis of influencing factors results showed that, the influencing factors of the hospitalized expenditure is various, including the hospital time, burden form, the hospital days, concomitant disease and the anesthesia ways, and so on. The hospital days number is an extremely important factor. Conclusions and recommendations(1) Receive the reasonable charge, and reduce the drugs expense, avoid the medicineabusing, cease the undesirable tendency; (2)Prevent the abuse of checking and testing, make the reasonable inspections , reduce the burden on the patients;(3) Change the status of the price inversion in technology and pharmaceutical services; (4)Strictly suppress the one-time materials, curb the prices of medical equipments and one-time medical consumable materials; (5)Enhance the capacity of doctors and nurses, strengthen the education of medical ethics; (6) Establish the community health service systems, so it can conveniently give habitants health services , which can reduce the hospitalized expenditure; (7)Use the science payment way of the medical expenditure, enhance the burette management of the idioplasmic sickness, control single idioplasmic sickness' expenditure; (8)Consume the medical service compensative mechanism, and adjust the medical service price, carry on the macroeconomic regulation and control the medical expenditure.
