| Nonneoplastic epithelial disorders of the vulva is a group of chronic diseases caused by pigment alteration and degeneration of female pudendum skin and mucosa .At present , with regard to the etiological factor and pathogenesis of nonneoplastic epithelial disorders of the vulva, internal and overseas scholars have done some related research, consists of local nerves and blood vessel functional disorder,metabolism functional disorder,autoimmunity disorder,hormone alteration,free radical influence,psychological factor and so on.Superoxide dismutase(SOD) is one of the most important antioxidase in human body,it can make superoxide anion(O2-) to disproportionate oxygen molecule and hydrogen dioxide. Otherwise,SOD also participates in apoptosis process,it can interact with some apoptosis factor, experiments indicate that TNF can induces producing free radical in cell, but SOD expressed in cell can inhibit apoptosis induced by TNF.There are some relationship between cell immune function and SOD.Because SOD activity drawdown,its capability of clearing oxyradical decrease,and leads to enhancing lipid peroxidation, initiates lipid peroxidation damage,direct affects cell membrane liquid state,fluidity and permeability , membrane functional disturbance,at last cause the whole cell immune function decrease.Either innocent process or malignant tumor of gynecology,there is different expression of SOD. Sugino N'research indicates that the expression of Mn-SOD regulated by TNF-αand PK-C in endometrial interstitial cell are related to NF-kcppa-B.This phenomenon may be a self- defence mechanism ofendometrial interstitial cell resisting oxidative damage induced by TNF-α. Therefore ,detect SOD level can be consider a referred index of diagnosising endometriosis and endometriosis interna. Sanchez M discovers that expression of SOD decreasing,it cues that change of oxidizing reaction may have relation with DNA damage of tumor cell . Senthil K points out the expression of SOD decreasing in ovarian cancer patients'blood plasma is because of it be used to clear lipid peroxidation matter and inhibit tumor cell.Our experiments use immunohistochemistry to detect the expression of SOD in nonneoplastic epithelial disorders of the vulva,and approach if oxidative damage play a part in local tissue.There is higher expression of SOD in normal vulvar tissue and squamous cell hyperplasia the vulva,and lower expression of SOD in lichen sclerosus of the vulva and vulva squamous of carcinoma . Normal vulvar tissue compare with lichen sclerosus of the vular ,there is significant difference of SOD expression(P<0.05). Vulva squamous of carcinoma tissue compare with squamous cell hyperplasia of the vulva ,there is significant difference of SOD expression (P<0.05).Squamous cell hyperplasia of the vulva compare with lichen sclerosus of the vulva, there is significant difference of SOD expression (P<0.05).Then suppose two types of this disease may cause by different etiopathogenisis,and the action path and mechanism of SOD in different types of disease is not completely alike.With the deeply recognizing of disease,we find estrogen participate in many regulation of physiologic function and patho-process of organism. Classic estrogen active pattern is connected with estrogen receptor(ER), regulate target gene transcription by its upstream estrogen response element.ER locate in cell nucleus or cytolymph, possessing action of transcription factor.Since Green had reported ER cDNA sequence,people have more understanding of ER molecule formation and function, approach the relationship of ER and development of breast cancer and ovarian cancer. Séverine Cunat demonstrated differential expression of ERαor ERβduring ovarian carcinogenesis, with overexpression of ERαas compared to ERβin cancer. Peter R. Lindgren pointed out a simultaneous expression of ERα, ERβand PR in epithelial cells of all histopathological tumor types was noted, with the notable exception of all mucinous tumors who remained ERβ-positive, but ERα- and PR-negative.Our experiment detected the expression of ER in normal vulvar tissue,squamous cell hyperplasia of the vulva,lichen sclerosus of the vulva,and vulva squamous of carcinoma.We find that there is higher expression of ER in normal vulvar tissue and squamous cell hyperplasia of the vulva,and lower expression of ER in lichen sclerosus of the vulva and vulva squamous of carcinoma. Normal vulvar tissue compare with lichen sclerosus of the vulva,there is significant difference of ER expression (P<0.05). vulva squamous of carcinoma tissue compare with squamous cell hyperplasia of the vulva, there is significant difference of ER expression (P<0.05). Squamous cell hyperplasia of the vulva compare with lichen sclerosus of the vulva, there is significant difference of ER expression(P<0.05).It indicates that the damage of estrogen system is slighter in squamous cell hyperplasia of the vulva.Nonneoplastic epithelial disorders of the vulva,cue estrogen in vivo still play a part in regulation of this type pathological changes,and offer some evidence of endocrine therapy.Otherwise,our experiment detecte there are both expression of SOD andER in normal vulvar tissue,nonneoplastic epithelial disorders of the vulva and vulva squamous of carcinoma.So we suppose the expression of them may have complicate dependability,especially both the two participate in apoptosis process, impact on eachther by apoptosis factor. More and more research indicate that ROS is not only toxin of organism,but also can affect signal transduction and gene expression in cell.The process of glutathione oxidase-1 and Mn-SOD induced by estradiol demand the participation of ER and its transduction path often involve in oxidative stress. Estrogen combine with ER,can increase the expression of favourable gene of human, include encoding antioxidase,SOD and glutathione oxidase. Therefore ROS in female chondriosome is fewer than male's,female can resist easily than male.Although oxidative damage and level change of hormone were regarded as two independent factor which leading to nonneoplastic epithelial disorders of the vulva had been known by people piecemeally,the complicated relationship between the two have not revoked sufficient reconstruction. Oxidative damage and level change of hormone may play a part in disease development collective.Therefore,understand the complicated relationship between the two can help us interpret much vital phenomena even disease development. |
