| Objective: To approach the applied value of ultrasound knife in practicing thyroid gland operation of little incisal opening endoscope .Metheds: We have analyzed hospital data of 5 innocence thyropathy patients who had been practiced thyroid gland operation of little incisal opening endoscope by ultrasound knife in 2005-2007(including base-court cases).Results:All the 5 operations were succeed.2 half single tumor operations,lasted 55min~80min, average 65min or so.1 both-side multiple tumor operations, average 160min or so.2 half multiple tumor operations, lasted 60min~140min, average 110 min or so.Volume of blood in operation: 5~30ml,average 15ml. Hospital day after operation: 1~7d,average 3.0d.Postoperative recovery were well, light pain,no infection of incisional wound and internal hemorrhage of tresis vulnus.Inflamed engorgement of cervical part appearance were limited professional jargon,with little degree.2 cases exposed sick-side recurrent laryngeal nerve,1 case combined low-grade recurrent laryngeal nerve paralysis,and recoveried at postoperative 6 weeks. Followed up all the 5 cases,3 month to1year,no recrudescence.Conclusions:ultrasoundknifewasanewtypeofsurgeryinstrument, applied to thyroid gland operation of little incisal openingendoscope. It couldincreaseoperationsafetyremarkably, coulddegradeoperationdifficulty notably. Compared to traditionary modusoperandi, practicingthyroid gland operation of little incisal opening endoscope by ultrasound knife had so much good qualities, such as tiny trauma, minor bleed, no thrum invisualfieldwhen operating, quickpostoperativerecovery, fewcomplication, short length of stay and soon. Inaddition, becauseoftheperitoneoscopic amplified action, anatomic structure weredistinct. Wecoul0d discriminate laryngealnerve and blood vessel structure clearly, thusavoidedtheirinjuries.Inaddition,whenpracticingcervicalperitoneoscopicoperation ,weshouldbefamiliartoneckdissectionandhaveallaroundexperienceoflaparoscopic operation. These are all the mostimportantfactors ofpateminutetrauma operation. |
