| Objective: To develop and improve the operative risk assessment software for orthopaedics surgeon to assess the operative risk of the elderly patients with hip fracture based on ORAS1 .Methods: ORAS2 was developed by VisualBasic6.0 and SQLServer2000 database , which contain five measuring scale including POSSUM,P-POSSUM,FRS,Harris and WOMAC. 31 patients (16men and 15 women) aged 60 years and more elderly (mean age 74.87±7.41) with hip fracture who were admitted to the orthopaedics department of the first affiliated hospital of Jilin university between January 2006 and March 2007 have been studied retrospectively with ORAS2. The preoperative physiological scores were obtained and multivariate analyses were performed. We also calculated FRS,Harris and WOMAC of patients who have undergone the surgery with ORAS2,and compared the fig between preoperative and that after operation. Finally, we use ORAS2 to make a decision for those elderly who were undergoing hip fracture.Results: There was no statistically difference between the patients with complication predicted by ORAS2 and the observed (13 and 11). Only 1 patients in this group died after operation. There was perfect agreement between the observed and the predicted morbidity as calculated by POSSUM equation. What`s more, there is a significant decline of fig of FRS,Harris and WOMAC after operation, that is 20.90±7.78, 19.17±6.63, and 21.42±10.66.Conclusions: 1.ORAS2 can be used to assess the operative risk of the elderly patients with hip fracture undergoing hip arthroplasty rapidly and conveniently;2. ORAS2 can predict the mortality and morbidity of patients and also can manage the hip function of patients;3.we can ORAS2 to make a decision of treatment perscriptionuse in the clinic. |
