Objective: To explore the clinical value of echocardiography, tissue Dopplerimaging(TDI) in assessing cardiac function damage in malignant patients receivingcardiotoxic anthracycline(ATC) chemotherapy.Methods: 50 patients were divided into two groups:â… group(the patients weretreated by anthracycline); andâ…¡group (the patients were treated by the otherchemotherapeutic drugs). These patients were examined by echocardiography, tissueDoppler imaging before chemotherapy and six months later. Besides the routineechocardiography parameters, the peak motion velocity during systolic period(Sm,Sm'), early diastolic period (Em,Em') and atrial contraction period(Am,Am') of mitralannulus and tricuspid annulus were obtained at apical four chamber view.Results: Parameters acquired from common echocardiography showed: leftventricular short-axis systolic function after chemotherapy had no statistic significancecompared with those data acquired before chemotherapy inâ… ,â…¡grouprespectively; Abnormalities of routine diastolic function index of left ventricle appearedin only 13 patients(9 patients inâ… group, 4 patients inâ…¡group), inclding the ratio ofE/A decreased (P<0.05) inâ… group; 7 of 13 patients had right ventricular diastolicfunction index abnormalities, Besides the ratio of E/A the other parameters of commonechocardiography were no significant difference between the two groups. TDI showed:after chemotherapy Em,Em' and the ratio of Em/Am,Em'/Am' decreased (P<0.01, P<0.05) while Am,Am' increased (P<0.01, P<0.05) respectively inâ… ,â…¡group;Sm,Sm' decreased (P<0.01, P<0.05) inâ… group compared with that acquired beforechemotherapy; The parameters of TDI were significantly difference between the twogroups(P<0.05).Conclusion: Compared with Echocardiography TDI is more sensitive in detectingabnormalities of heart function, it can be used in assessing early heart damages inducedby anthracycline chemotherapy.
