Objective: Through surveying occupational stress and mental health status of engine drivers selected, measuring their cortisol and interleukin-6 concentration in blood serum, the aim is to study the effect of occupational stress on mental heath and certain biochemical indices of engine drivers, to bring forward a general measure to ease occupational stress and protect engine drivers from bodily and mental illness,to provide management-improving basis for safety control section and to ensure the safe running of railway transportation.Method: Taking the chance of collective health checkup for engine drivers, we selected 280 in-service engine drivers (except yardmasters) in Taiyuan Locomotive Service Zone as studying objects. After the surveyors explained the surveying purpose, significance, fill-in method and standard, three forms of questionnaire (general condition, OSI and SCL-90) are handed out to each of these engine drivers and filled out by them ,and then checked on the ground one by one by the surveyors. At the same time, take empty-stomach intravenous blood samples from these drivers and measure cortisol and IL-6 concentration in blood serum using radio-immunizing method.Result:1. Compared with that of general population , the SCL-90 total mark, the number of items detected positive and most of factorial marks of engine drivers are higher, indicating these engine drivers surveyed having multifarious mental health problems, remarkably putting up somatization and hostility, secondarily, compulsion, depression, anxiety and psychotic mentality.2. The main factors of affecting engine drivers' mental health are occupational stress from family & job, experience & achievement, social relation, and physical exercise, entertainment activity, present post-serving period, age and average monthly wages per person of family, among which occupational stress factors take the most account. Occupational stress is directly proportional to SCL-90 total mark and factorial mark.3. The SCL-90 total mark and 9 factorial marks differ remarkably among different degree occupational stress surveying groups. 4. Canonical Correlation Analysis: anxiety, strain social relationship, bigotry, depression of engine drivers are mainly caused by occupational stress from family & job, experience & achievement, social relation, and physical exercise, entertainment activity, age and present post-serving period etc.5. Occupational stress from social relation is the influential factor that causes engine driver's IL-6 content to rise. Cortisol concentration in blood serum of freight train drivers is higher than that of passenger train drivers.Conclusion:1. In the occupation of engine driver exist many factors that will cause strain, and these factors may bring some mental health problems. Occupational stress from social relation is the influential factor which can raise engine driver's IL-6 concentration in blood serum. Cortisol concentration in blood serum of freight train drivers is higher than that of passenger train drivers.2. With regard to the main Occupational stress factors and mental health problems, it is very important to take comprehensive measures to intervene effectively. Periodic mental health consultation and instruction shall be conducted for engine drivers of different ages and service periods, and those who have mental health problems should be given corresponding psychotherapy. Practicable and effective health care measures should be made and carried out, such as rest and recuperation, tour and traveling, enriching literary and physical exercises, so as to relieve the strain and help healthy. Labor protection should be reinforced, working environment improved and proper working schedule and resting holidays arranged to ease off engine drivers' tiredness from work. Meanwhile, vocational skills training and safety education should be reinforced to buildup technique ability and consciousness for safety, ensuring safe running of railway transportation.
