Acute necrotizing pancreatitis (ANP) is one of the common acute clinical abdominal diseases. Because its pathogenesis is unclear, there are no effective measures to treat it, so its mortality is very high. Recent researches show that over-activation of white cell causes a lot of inflammatory mediators releasing. It can cause systemic inflammatory response syndrome (SIRS) and multiple organs dysfunction syndrome (MODS) which is the main reason of death for patients at the early stage of ANP. At present nuclear transcription factor-kB (NF-kB) is considerated the"switch off"of inflammatory reaction. NF-kB enters into the nuclear when it is activated, then transcribe a lot of inflammatory mediators: Tumor necrosis factor-α, interleukin-1β, interleukin-2, interleukin-8 , monocyte chemotactic peptide, intercellular adhesion molecule-1 and vascular cell adhesion molecule-1 which is the main causes of multiple organs damage of ANP. It would be a good way that NF-kB is inhibited by some drugs to treat ANP. The study that panax notoginseng saponins treats ANP by inhibiting activation of NF-kB is few. [Objective]Our research mainly demonstrates: 1.Changing of NF-kB activition have relation with the damage of pancreas and lung; 2.Does panax notoginseng saponins, the inhibitor of NF-kB, decrease the function and pathological injury in the ANP rat?[Methods] Thirty Sprague-Dawley rats were randomly divided into three groups: Sham operation group(group SO), acute necrotizing pancreatitis group(group ANP), panax notoginseng saponins preconditioning group (group PNS). There were 10 rats in each group. Rat acute necrotizing pancreatitis model was induced by retrograde injection of 5% sodium taurocholate to pancreatic duct (0.1 ml/100g). The rats in the panax notoginseng saponins preconditioning group were given by intraperitonel injection of 0.1 ml/100g panax notoginseng saponins(50mg/ml) one hour before model establishment, and those in the other two groups were given 0.9% physiological saline. Put rat to death at 6 hour after the model were made, measured serum level of AMY, ascites volume, pulmonary and pancreatic wet/dry ratio. The serum levels of AMY were detected with biochemical analyzer. Tissues from pancreas and lung were obtained to odserve the changes with microscope. Pancreatic and pulmonary tissue were made to detected the activity of NF-kB by immunohistochemistry.[Results]1.The ascites volume, serum AMY levels and pancreatic and pulmonary wet/dry weight ratio: The ascites volume, serum AMY levels, pancreatic and pulmonary wet/dry weight ratio of ANP group were significantly higher than the SO group after model establishment at 6 hour. The ascites volume, serum AMY levels, pancreatic and pulmonary wet/dry weight ratio of PNS group significantly decreased compared to SNP group. (P all <0.05)2.Pathological changes and histological grading of pulmonary and pancreatic tissue: Pathological changes of pulmonary and pancreatic tissue in rat of PNS group obviously alleviated compared to SNP group. Histological grading of pulmonary and pancreatic tissue in ANP group was significantly higher than the SO group after model establishment at 6 hour. Histological grading of pulmonary and pancreatic tissue of PNS group were significantly less than in SNP group(P<0.05) .3.NF-kB activity of pulmonary and pancreatic tissue: The nurmber of positive cells and positive unit of pulmonary and pancreatic tissue in SNP group emarkably increased compared to SO group after model establishment at 6 hour(P<0.01) , and they decreased much more in PNS group than that in ANP group(P<0.05) .4.Analysis of correlation: NF-kB activity of pulmonary tissue in ANP group had positive cirrelation with the score of lung injury and pulmonary wet/dry weight ratio. The correlation coefficient were 0.73 and 0.87, respectively(P<0.01) . NF-kB activity of pancreatic tissue in ANP group had positive cirrelation with the score of lung injury and pulmonary wet/dry weight ratio. The correlation coefficient were 0.75 and 0.84, respectively(P<0.01) . NF-kB activity of pancreatic tissue in ANP group had positive cirrelation with the score of pancreas injury, pancreatic wet/dry weight ratio and ascites volume. The correlation coefficient were 0.75, 0.64 and 0.78, respectively(P<0.01) .[Conclusion] 1.NF-kB is activated obviously in pancreas and lung in rat acute necrotizing pancreatitis; 2.Activating of NF-kB shows position relationship with the damage of pancreas and lung. Those show that activated of NF-kB related to the MODS in rat acute necrotizing pancreatitis; 3.Inhibiting of NF-kB by panax notoginseng saponins, and pathological damage of pancreas and lung were also reduced. Those show that ANP might be treated by panax notoginseng saponins in the future.
