Objective: Diabetic Nephropathy (DN) is a specific kind of kidney damageof Diabetic Mellitus (DM), and it can further develop kidney failure, whichis one of the most lethal reasons for DM. The effect of lipodystrops onkidney related diseases draws attention from most kidney related diseaseexperts. Through years of clinical observations, we have found that themorbidity of "Qi and Yin deficiency,blood stasis and dampness" typeDN is 70%. With the hot and moist weather in the Lin-Nan area, patientsare more vulnerable to the abnormal damp-hot, therefore, DN patientsmostly carry hot and dampness. It is difficult to treat becausestrengthening body resistance may keep pathogen, and eliminating pathogenmay hurt healthy energy. The objective of this study is to observe thecurative effect on early-mid phase DN patients with the use ofsupplementing Qi. and nourishing Yin, eliminating damp and blood stasischinese medicines.Methods: One hundred early-mid phase DN patients from in-and-outendocrinology department in the 2nd Affiliated Hospital of TraditionalChinese Medicine University were randomly divided into two groups, whichwere the control group and the treatment group. These two groups were bothtreated with Gliquidoneand, or insulin and Acarbose to control bloodglucose level. Blood pressure(BP) control was based on the BP level toselect Aprovel(Irbesartan Tablets)150-300mg/day, or Diovan (valsartan)80-160mg/day; if BP was not reduced to 130/90mmHg or lower,Plendil 5-10mg/day would be added. Blood fat control was to use Lipitor10-20mg/day. On the basis of the above treatment, the treatment group weregiven Qi-reinforcing and Yin nourishing,blood stasis and dampnesseliminating chinese medicine. The course of treatment was consisted of3 months.Results: After the treatment, fasting plasma glucose (FPG),2h plasmaglucose(2hPG) and glycosylated hemoglobin(HbAlc) were both lowereddrastically after the treatment(P<0.01), however, the treatment grouphad better results(P<0.05). The treatment group also showed greateroutcome than the control group in lowering 24h urinary protein,bloodcreat (Scr),blood urea nitrogen (BUN), and improving blood coagulation (P<0.05). The total cholesterol(TC),trig]yceride(TG),low densitylipoprotein-cholesterol (LDL-C),the high densitylipoprotein-cholesterol(HDL-C) levels of both groups were all improvedafter the treatment(P<0.01 and P<0.05), and the treatment group showedbetter improvement(P<0.05). All the above changes indicated that thetreatment group showed better improvement than the control group(westernmedicine only) on improving kidney function,reducing urinary protein,regulating fat digestion, with the combined use of Qi-reinforcing and Yinnourishing,blood stasis and dampness eliminating chinese medicine andthe western medicine.Conclusion: Qi-reinforcing and Yin nourishing,blood stasis and dampnesseliminating chinese medicine can have curative effect on treatingearly-mid phase DN. Furthermore, it would be better to combine with theuse of western medicine in order to produce greater outcome, like reducingurinaryprotein and improving kidney function.
